The Herbal Remedy Which is Better Than Medicine Fludrocortisone and Midodrine For Treatment of Orthostatic Hypotension (Postural Hypotension)

Herbal butcher's broom (Ruscus aculeatus) may be considered as a replacement of medicine such as fludrocortisone and midodrine for treatment of orthostatic hypotension (OH), a recent study suggested.

Orthostatic hypotension (OH) is also known as postural hypotension, a condition happens when a person stands up from sitting or lying down or in sudden adjust of body position, the blood pressure falls, and a person may be experience lightheaded or sometime pass out.

What causes the blood pressure to fall when a person adjust positions suddenly from sitting position are debatable. Some researchers suggested that dehydration, blood loss, and anemia are the most common conditions for a person to develop low blood pressure.

Orthostatic hypotension (OH) mostly affects the groups of the aging population with autonomic insufficiency, such as diabetes, Parkinson's disease, and chronic fatigue syndrome; and anyone on drugs that affect the autonomic nervous system. 

In conventional medicine, most cases of orthostatic hypotension (OH) are normally treated with fludrocortisone and midodrine.

Butcher's broom is a low evergreen shrub, a species of Ruscus aculeatus, belonging to the family Asparagaceae, native to Eurasia.

The herb  has been used in herbal medicine to treat reduce inflammation, peripheral vascular disease, hemorrhoids, bronchial asthma, varicose veins, hypotension, jaundice, functional disorders of the heart, kidney disorder, venotonic,..... and anti-edematous properties.

The Chemical Ingredients
It contains rutin, sitosterol, sparteine, spirostanol saponins, steroidal sapogenins, sterols, stigmasterol, sulfated steroidal saponins, sweet broom, tetracosanoic acid, triterpenes, triterpines, tyramine, etc.

In the study to find the natural ingredient which have therapeutic effect in replaced pharmacologic and nonpharmacologic agents for treating OH reveals with disappointing and marginal activity, researchers at the American University found that

* Ruscus aculeatus is an natural alpha-adrenergic agonist that induces venous constriction by directly activating function postjunctional alpha1- and alpha2-receptors in stimulating the release of noradrenaline at the level of the vascular wall to provide more blood to sympathetic nervous system.
These results may have a direct effect in reduced  risk of fall of the blood pressure.

Noradrenaline, a catecholamines plays major roles in regulation of function of the brain.

* Flavonoid, the major ingredient of ruscus aculeatus was also found to strengthens blood vessels, reduces capillary fragility, thus maintaining healthy circulation

* Most importantly, unlike most of the drug therapies used to treat OH, Ruscus aculeatus does not cause supine hypertension

The findings suggested that Ruscus aculeatus, a phytotherapeutic agent containing ruscogenins and flavonoids, may prove useful for the treatment of OH due to its potentially compromised receptor activity at the venous wall.

After taking into account of other con and cofounders, Dr. Redman DA at the final report said, "It also appears to do something no other therapy can offer--alleviate the worsening effects of OH in environmentally hot conditions. Finally, it is an extremely safe, inexpensive, over-the-counter botanical medicine" and "With proven phlebotherapeutic properties, including vasoconstrictive action and venotonic properties, Ruscus aculeatus shows great promise for ameliorating the symptoms of OH and improving the quality of life for large groups in the population. It clearly deserves to be the object of wider research and study as a treatment for OH".

Taking altogether, herbal Butcher's broom may be considered as functional herb for treatment of orthostatic hypotension (OH) with no adverse effect in compared ineffectiveness of conventional medicine,
