Omega 3 Fatty Acids, the Best Antioxidant for the Treatment of Hypertriglyceridemia

Hypertriglyceridemia is a condition characterized by high levels of triglycerides in the blood.

Triglycerides are accumulated in the blood for future use by converting any unused calories.

In other words, triglycerides are an ester derived from glycerol and three fatty acids (from tri- and glyceride).

Overexpression of hyperglyceridemia is found to patients with atherosclerosis even without the presence of high cholesterol levels, and predispose.

Hypertriglyceridemia can also be accumulated to the arterial wall or combined with other substances to form plaque that narrows the arteries in transporting rich oxygenated blood to the heart and other parts of the body.

There are 2 main sources of plasma triglycerides
* Dietary fat which is carried in chylomicrons.

* Fat is produced by the liver carried in very-low-density lipoprotein (VLDL) particles.

Primary hypertriglyceridemia is found in 5% of patients associated with genetic preposition compared to 95% of secondary hypertriglyceridemia found in obese metabolic syndrome, diabetes,... patients.

Treating hypertriglyceridemia is difficult, as the condition is frequent coexistence with other conditions that affect cardiovascular disease risks, such as high levels of low-density lipoprotein, obesity, metabolic syndrome, and type 2 diabetes.

In severe cases of hypertriglyceridemia, patients may also experience the complication of acute pancreatitis, independent of the risk of CVD.

Omega-3 fatty acids are phytochemicals in the class of lipids, found abundantly in deepsea fisk, dark-green leafy vegetables, grains, legumes, nuts, etc.

The three main components of Omega-3 fatty acids are alpha-linolenic acid (ALA), eicosapentaenoic acid (EPA), and docosahexaenoic acid (DHA).

On finding a potential phytochemical for the treatment of chronic diseases associated with obesity, researchers examined the three omega-3 fatty acids (OM3FA) prescription formulations that are approved in the US for the treatment of adults with severe hypertriglyceridemia.

* (1) OM3FA ethyl esters (OM3EE), a mixture of OM3FA ethyl esters, primarily eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) (Lovaza®, Omtryg™, and generics)

* (2) Icosapent ethyl (IPE), EPA ethyl esters (Vascepa®); and

* (3) omega-3 carboxylic acids (OM3CA), a mixture of OM3FAs in free fatty acid form, primarily EPA, DHA, and docosapentaenoic acid (Epanova®).

According to the tested analysis, although all 3 approved formulations showed a strong effect in the inhibition of diacylglycerol acyltransferase, increased plasma lipoprotein lipase activity, decreased hepatic lipogenesis, and increased hepatic β-oxidation,.OM3FAs appears to be an effective treatment option for patients with severe hypertriglyceridemia.

Most importantly, all three formulations are well tolerated (the most common adverse events are gastrointestinal) and demonstrate a lack of drug-drug interactions with other lipid-lowering drugs, such as statins and fibrates.

Taken altogether, Omega-3 fatty acids may be considered a remedy for the prevention and treatment of hypertriglyceridemia, pending to the confirmation of the larger sample size and multicenter human study.

Intake of Omega-3 fatty acids in the form of supplements should be taken with extreme care to prevent overdose acute liver toxicity.
