Good news to coffee lovers, coffee and coffee caffeine consumption daily and regularly might have a potential in reduced risk of gastric cancer, the recent study suggested.

According to the joint study led by the the Children's Hospital of Chongqing Medical University, by searching of the data base of published literature, including 22 studies (9 cohort and 13 case-control studies) involving 7,631 cases and 1,019,693 controls, the relative risk of gastric cancer and coffee intake was shown in following
1. Relative rsik of gastric cancer was 0.94 (95% CI: 0.80-1.10) for the highest category of coffee consumption compared with the lowest category
2. In compared to non drinker, the relative risk is and 0.93 
3. The relative risk of <1 cup/day, 1-2 cups/day and 3-4 cups/day coffee consumption compared with nondrinkers were 0.95, 0.92 and 0.88, respectively,

The information findings, after considering other factors, demonstrated a significant association between coffee intake and decreased gastric cancer risk in case-control studies.

Contraditively, in the study of long term effect of coffee consumption and risk of gastric cancer in US, through searching PubMed and Embase from January 1996 through February 10, 2015 of a total of 2019 gastric cancer incident cases among 1,289,314 participants with mean follow-up periods ranging from 8 to 18 years, suggested that
1. The relative risk (RR) of every 3 cups/day increment of total coffee consumption was 1.07
2. the relative risk of 1.18 in compared with the lowest and the highest categories of coffee consumption
3. RR of 1.06 in compared the second highest and Lowest category (median 3.5 cups/day),
4 RR of 0.97 (95% CI = 0.79-1.20) for the third highest and lowest category (median 1.5 cups/day).

Dr., the lead author said, "Current evidence indicated there was no nonlinear association between coffee consumption and gastric cancer risk. However, high coffee consumption (more than 6.5 cups/day) might increase the risk of gastric cancer in the US population".

Other, subgroups analysis suggested that coffee intake do not have potential effect in gastric risk, but in US subgroup, risk of gastric significant increased risk with the relative ratio of 1.36 in the studies with <10 years of follow-up, particular in those studies without adjustment for smoking.

Taking altogether, although there is conflict evidences, coffee consumption < 3 cups/day was shown to ameliorated risk of gastric cancer.

In non smokers, according to the pooled data set comprises 564 cases and 2929 hospital or population controls enrolled in ten studies conducted in Denmark, Germany, Greece, France, Italy and Spain. risk of bladder caner did not increase monotonically with dose but for heavy coffee drinkers of over 10 cups per days, the risk bladder cancer increases substantially.

Dr. Sala M, the lead author said, "Nonsmokers who are heavy coffee drinkers may have a small excess risk of bladder cancer".

Other, in the analysed a case-control study in two different areas of northern Italy (555 cases and 855 controls), coffee intake and risk of bladder cancer showed that

1. RR 1.2 for one cup of coffee per day,
2. RR 1.4 for two,
3. RR 1.5 for three and
4 RR 1.4 for four or more
5. RR 1.5 (0.9-2.4) for decaffeinated coffee.
But in never smoked participants, the risk of bladder cancer increased significantly

3 RR 1.5 for four or more

An oral glucose tolerance test identified 55 men and 52 women with previously undiagnosed type 2 diabetes and 172 men and 167 women with impaired glucose tolerance, researchers at the Karolinska Institutet,file the following results
1. The relative risks when drinking >/=5 cups of coffee per day compared with </=2 cups per day in men were 0.45 and 0.63, respectively, and after adjustment of type 2 diabetes and impaired glucose tolerance but in women, the relative risk was reduced substantially to  0.27.
2. Coffee and coffee caffeine intake was associated inversely in patients with type 2 diabetes.
3. Women, but not in men, with impaired glucose tolerance also benefit from improvement of beta-cell function
4. Both gender with normal glucose tolerance, coffee consumption also displayed a strong effect in reduced risk of insulin resistance.

Dr. Agardh EE, the led author said, "high consumers of coffee have a reduced risk of type 2 diabetes and impaired glucose tolerance. The beneficial effects may involve both improved insulin sensitivity and enhanced insulin response".

Furthermore, in rats, radiotelemetry transmitters with electroencephalogram and electromyogram electrodes implanted for sleep recording in animals administered with caffeine alone (10 mg/kg) or in combination with zolpidem (10 mg/kg) or trazodone (20 mg/kg), or vehicle, in crossover experiments with 12 healthy male volunteers in a randomised, placebo-controlled, 4-week crossover study, caffeine injection group showed a significant prolongation in objective sleep onset latency (SOL) with disruption in subjective measures of sleep onset caused by caffeine in rats and humans in compared to other groups.

Taking together, coffee and coffee caffeine may have disruptive effect in induced insomnia and symptoms of insomnia, particularly in female,  therefore women who have a history of insomnia should reduced intake of coffee or only drink them in the morning at reduced amount.

Dr. Eskelinen MH, the lead author said, " In the CAIDE study, coffee drinking of 3-5 cups per day at midlife was associated with a decreased risk of dementia/AD by about 65% at late-life".

Progressively, the midlife coffee and tea drinking and risk of Aging and Dementia (CAIDE) study, suggested that people drinking 3-5 cups per day in their midlife may benefit to a decreased dementia risk of over 65%.

More importantly, the follow up follow-up of 21 years, 1409 individuals (71%) aged 65 to 79 completed the re-examination in 1998 of a randomly selected from the survivors of a population-based cohorts previously surveyed within the North Karelia Project and the FINMONICA study in 1972, 1977, 1982 or 1987 (midlife visit). also postulated that coffee and coffee caffeine intake at midlife showed a significantly lower risk of dementia and AD later life in compared with those drinking no or only little coffee after adjusted for other factors.

Laryngeal cancer is a medical condition of irregular cell growth in the tissues of the larynx. The cancer cells may spread to other tissues and organs in the late stage.

In the review literatureof a total of 2,803 cases and 503,234 controls in 10 independent studies identified by searching electronic database (Medline and EMBASE) and reviewing the reference lists of relevant articles until Oct. 2013, researchers found that incidence of larygeal cancer was associated to coffee consumption, in compared to non drinkers. Intake of coffee showed a significant relative risk of RR = 1.47 in compared to base line.

Interestingly, researchers found, no statistically significant association of cancer incident difference in comparison between moderate and low coffee drinking .

 According to the study, there was a evidence of statistically significant association of dose of coffee consumption and risk of laryngeal cancer. 

Dr. Chen J, the lead researcher said, " A dose-response relationship between coffee intake and laryngeal carcinoma was detected" and "this meta-analysis of observational studies demonstrate that coffee consumption would increase the laryngeal cancer risk".

According to the result published, the study also identified that among the 6 studies that reported the association between coffee consumption and laryngeal carcinoma, only one study was a prospective population based cohort study, while the rest 5 studies were retrospective hospital based case-control studies using observation of returned survey from participants.

Contrastively, also in the searching of data base PubMed, Web of Knowledge and Wan Fang Med Online, identified 8 studies including 2167 laryngeal cancer cases, researchers insisted that there was insignificant association between coffee and coffee caffeine intake and risk of laryngeal cancer, regardless to volumes intake.

Taking together, coffee and coffee caffeine intake may have a negative effect in increased risk and laryngeal cancer but researchers suggested that  genetic and environmental factors  such as alcohol intake, human papillomaviruses infection, and silica exposurem should be also taken  into account. which were excluded in some studies before expressing.

In an acute randomized, controlled, cross-over human intervention trials conducted to examine the impact of coffee intake, matched for caffeine but differing in CGA content (89, and 310 mg) on flow-mediated dilatation (FMD) in 15 healthy male subjects, in compared a second intervention trial conducted with 24 healthy male subjects, the impact of pure 5-caffeoylquinic acid (5-CQA), the main CGA in coffee(5-CQA; 450 mg and 900 mg) on FMD, researchers filed the following results
1. Low and high CGA(89 mg and 310 mg CGA) intake, showed a significant effect on vascular function 1 hour after injecting coffee
2. Coffee intake was found closely associated to the paralleled to the appearance of CGA metabolites in regulated the normal function of arteries and veins at 5 hours
3. 5-CQA (450 mg) expressed an improvement by FMD assay after i hour of administration.

The information findings suggested that coffee has a strong effect in activated vascular function through its phytochemicals, including CGA and its metabolites in plasma, 3-, 4- and 5-feruloylquinic acid and ferulic-4'-O-sulfate at 1 h and isoferulic-3'-O-glucuronide and ferulic-4'-O-sulfate. 

Other, in a total of 38 men and 37 women with a mean ± SD age of 38.5 and body mass index of 24.1 randomly assigned to 3 groups: a control group that did not consume coffee or a placebo and 2 groups that consumed 400 mL coffee/d for 8 wk containing a medium (MCCGA; 420 mg) or high (HCCGA; 780 mg) CGA content, researchers filed the results as follow
1. Coffee consumption over the period indicated and after 1 hours showed a significant presence of caffeic and ferulic acid concentrations in the coffee-drinking groups, with different values in 2 coffee injection group.
2. In the same time as in 1, the levels of antioxidant expression increased substantially in 2 coffee intake group, but undetectable in control group or placebo.
3. All amount of antioxidants and phytochemicals creased to exist after 8 weeks.

The evidences in the study demonstrated a acute effect of coffee in promoting of vascular function in improved nutrients and fluids transportation by arteries and vessels.

Dr. Agudelo-Ochoa GM, the lead author said, "Both coffees, which contained CGAs and were low in diterpenes and caffeine, provided bioavailable CGAs and had a positive acute effect on the plasma AC in healthy adults and no effect on blood lipids or vascular function" and "The group that did not drink coffee showed no improvement in serum lipid profile, FMD, BP, or NO plasma metabolites".

Taking together, there is no doubt that coffee consumption have an enormous effect in improved vascular function in reduced risk of cardiovascular incidence and complications.
