#Bromelain Combined with #Diclofenacgroups in Reducing Pain and Swelling and Quality of Life in Patients after Surgical Removal of Impacted LowerThird Molars

Pain and swelling is a natural response of the immune and nervous system after a wound or injury by inducing the production of proinflammatory cytokines.

In acute of infection, after sensing a wound and injury to the body, the immune first lines of white blood cell defense stimulate the production of blood palette to the infected area with an aim to cover the wound or injury and the inflammatory cytokines to kill off organisms trying to enter the body through the opening.

Please note that overexpression of proinflammatory cytokines in this phase has been found to cause abnormal healing with the formation of scars.

The acute phase of infection arrives and leaves quickly as it is resolved quickly by the body.

However, chronic infection is a result of an immune system that can not kill off all infectious organisms in the acute phase of infection, leading to chronic inflammation that last a long time, such as months or years.

Swelling and pain in the joints are one of the most common symptoms in patients with arthritis, particularly in rheumatoid arthritis (RA).

However, some cases of swelling accompanied by symptoms that do not associate to the injury such as fatigue, loss of appetite, and weight gain may be an indication of cancer, heart, liver, or kidney disease.

In most cases, pain and swelling of the injury and wound can be healed by themselves without interfering or injection of counter-pain medication, depending on the size of the opening.


Bromelain, a proteolytic enzyme found in pineapples (Ananas comosus) has been used in traditional medicine as an inflammatory agent and to treat pains, strains, muscle aches and pains and ease back pain and chronic joint pain, skin diseases, etc.

With an aim to find potential and natural compounds for the treatment of pain and swelling with no side effects, researchers evaluated the effect of bromelain after the surgical removal of third molars.

The prospective randomized clinical study included 40 patients requiring surgical removal of impacted mandibular third molars.

After the surgery, according to the tested assays, bromelain showed a significant effect in reducing swelling and pain in 28 out of a total of 40 patients.


Based on the findings, researchers said, "Bromelain can be used as a successful oral enzyme therapy for oral surgery patients, especially after removal of impacted third molars".

In order to reveal more information about bromelain anti-pain and swelling activity the team at the University of Mosul College of Dentistry compared the effect of oral bromelain (4 × 250 mg) versus oral diclofenac sodium (4 × 25 mg) on pain, swelling, trismus, and quality of life (QOL) after surgical removal of impacted lower third molars.

The randomized, double-blind, placebo-controlled study included 45 patients requiring extraction under local anesthesia of a single partial bony impacted mandibular third molar randomly divided into 1 of 3 groups: bromelain, diclofenac, and placebo.

According to the results of the analysis, bromelain and diclofenac groups both showed a significant and similar reduction in pain compared with the placebo group at all intervals.

Both bromelain and diclofenac treatment groups also showed a significant difference in the effect on QOL in most subscales and total scores.

Dr. the lead scientist said, "The results of our study have shown that oral bromelain is an effective therapy to improve the QOL after surgical removal of impacted lower third molars, with an effect on the postoperative sequelae comparable to that of preemptive diclofenac sodium".

Taken together, bromelain may be considered supplements for the and treatment of pain and swelling, pending the confirmation of the larger sample size and multicenter human study.


Intake of bromelain in the form of a supplement should be taken with extreme care to prevent overdose acute liver toxicity.
