***Alternative Therapy: Yoga In Regulation of Glucose Metabolism(Revised)

Glucose metabolism is a natural process of our body to convert carbohydrate intake into energy.

Yoga practice may have a positive and profound impact in improved glucose metabolism, thus reducing risk of early onset of diabetes. 

Yoga, the ancient technique for harmonized external and internal body well beings, through breath control, meditation, bodily movement and gesture..... has been best known for people in Western world and some parts in Asia due to health benefits reported by various respectable institutes' research and supported by health advocates.

In a prospective, randomized, intervention controlled trial with 90 adolescents aged between 15 and 18 years who met the Rotterdam criteria for PCOS. assigned either to yoga group practiced suryanamaskara, asanas, pranayama, and meditation 1 hour per day each day for 12 weeks or practiced conventional physical exercises, researchers found that
1. Yoga intervention group expressed a similarly significant improvement in fasting insulin, fasting blood glucose in compared to control and
2. In  homeostasis model assessment, yoga group showed a strong impact in insulin resistance in compared to control

Polycystic Ovarian Syndrome(PCOS) is an endocrinologic diseases among reproductive-age women caused by undeveloped follicles clumping on the ovaries that interfere with the function of the normal ovaries and associate to an increased risk for metabolic disorder.

Homeostatic model assessment (HOMA) is a method used to measure the levels of insulin resistance and beta-cell function.

After taking into account of other co founders, Dr.Nidhi R, the lead author said, "Yoga was found to be more effective than conventional physical exercises in improving glucose, lipid, and insulin values, including insulin resistance values, in adolescent girls with PCOS independent of anthropometric changes".

Further analysis also suggested that yoga intervention may have a regional glucose metabolism through expression of a slight increase of frontal rCMRGlc and a more pronounced reduction in primary and secondary visual centers.

More importantly, in a controlled trial within an university-affiliated hospital. 173 Chinese men and women aged 18 or above were assigned to either the yoga intervention group (n = 87) or the control group (n = 86) found that throughout analysis involving the entire study population revealed that the yoga group achieved greater decline in fasting glucose in compared to control.

After considering of other con founders in the study, researchers opinionated that A 12-week Hatha yoga intervention improved glucose and lipid metabolic risk profiles and HRQoL in Chinese adults with and without MetS.

Taking altogether, Yoga may be used as a therapeutic intervention in reduced risk of abnormal glucose metabolism, thus reducing risk of diabetes.

Flavanols (polyphenols) are rich phytochemicals isolated from green tea, particularly in the expression in form of most bioactive flavanol EGCG.

Oxidative phosphorylation (OXPHOS) protein ATP synthase-β (AS-β) plays a key role in energy production and regulated the generation of free radicals, and apoptosis through respiratory chain.

Mitochondrial DNA (mtDNA)-encoded OXPHOS protein NADH dehydrogenase-3 (ND3) in reduced risk of gene mutation through oxidative phosphorylation (OXPHOS).

Dr. Rehman H, the lead author said," Green tea polyphenols markedly attenuated CsA-induced renal injury and improved renal function. Together, these results demonstrate that green tea polyphenols attenuate CsA-induced kidney injury, at least in part, through the stimulation of MB".

In support of the efficacy of green tea for treatment of renal disease, the study of F40 rats randomly divided into 4 groups. Group 1 acted as control; group 2 received contrast medium (intravenous iodixanol, 10 mL/kg, as a single dose); group 3 received contrast medium and then green tea extract for 3 days (10 mg/kg/d, intraperitoneal); and group 4 first received green tea and then contrast medium, researchers showed that group treated with green tea had significantly higher overall scores than the control group.

Treatment group also expressed a significant improvement in progression of renal degeneration caused by nephrotoxicity of contrast media. 

Unfortunately, according to the joint study lead by the China Medical University,  inhibited the elimination of indoxyl sulfate (IS) and p-cresyl sulfate (PCS), a highly protein-bound nephro-cardiovascular toxins in CRF rats.

Catechin, is another major chemical constituent isolated from Green tea (GT) and a popular beverage in China.

Indoxyl sulfate(IS), a circulating uremic toxin has found to associate to the development of glomerular sclerosis and interstitial fibrosis.

P-Cresyl sulfate (pCS) is also an uremic toxin generated by intestinal bacteria
which has an implication to worsen outcome of patients with chronic kidney disease and increased cardiovascular burden.

Furthermore, green tea injection of catechin.in experiment mice also showed a significantly increased the levels of serum creatinine (Cr) and blood urea nitrogen (BUN), an indication of further kidney damage after ingestion of green tea catechin.

Dr. Peng YH, the lead author said, "GT inhibited the elimination of nephro-cardiovascular toxins such as IS and PCS, and deteriorated the renal function in CRF rats".

Taking together, the conflict results of green tea in expression of chronic renal disease may be due to study design, the group sample, the subject selection doses administration,........., therefore patients with renal disease should be consult with specialists before taking.

In the study of total of 20 subjects diagnosed with panic disorder, randomly assigned to both experimental groups: Group 1 (G1-Yoga: 10 subjects) attended yoga classes and Group 2 (G2-CBT + Yoga: 10 subjects) participated in a combined intervention of yoga practice followed by a cognitive-behavioral therapy (CBT) session, reesarchers filed the following results
1. Both group displayed a statistical analysis in reduction of anxiety levels associated with panic disorder, panic-related beliefs and panic-related body sensations
2. The combination of yoga and CBT (G2) showed a better reductions in all observed parameters in compared to only yoga group.

Cognitive-behavioral therapy is a psychological method of treatment to eliminate conception of negative patterns of thought about the self and the world,particularly in patients with depression.

Dr. Vorkapic CF, the lead author, after taking into account of other con-founders said, "... improvements in different mental health parameters after the practice of contemplative techniques alone or combined to psychotherapy" and "(Understanding) joining psychological and physiological variables could help better elucidate the mechanisms through which mind-body practices work to improve mental health".

In deed, yoga improved psychological and physiological variables of patients with Panic and Anxiety Disorder may be attributed to the increased level of gamma-aminobutyric acid, or GABA, a chemical in the brain in regulated nerve activity. Thus reducing psychological aspects in related mood and anxiety disorders..

According to the Emory University, gamma-aminobutyric acid can be considered as neural basis of fear acquisition, the mechanisms of fear inhibition or suppression


Other, in the review of literature published on quality of life in panic disorder from 1980 to 2010 on database of MEDLINE, PsycINFO, and PubMed, indicated that yoga intervention in combination of standard treatment may ameliorate substantial quality of life  impairments in panic disorder, improve mental and physical health.....and be used as post-treatment in enhanced quality of life (QOL) in these group of patients.

Dr. Davidoff J, the lead author went even further to suggest, " understand the nature of comorbidities in panic disorder (PD) as well as to determine whether additional interventions that have been studied in other psychiatric disorders, such as exercise, meditation, yoga, humor, massage, and nutritional supplements, can be utilized to improve QOL in PD to normal community levels.

Interestingly, in the reviewed literature published on PubMed in English up to December 2012 to evulaute the effect of complementary and alternative therapies in panic and anxiety disorder, researchers also found that
1. Data based illustrated that depression evidence base is significantly larger than anxiety disorder
2. Yoga was considered as levels 3 line of treatment of patients with panic and anxiety disorder due to the quality of available evidence.

Taking altogether, there is no doubt that yoga may be considered as an adjunct exercise in combined with standard therapy for treatment of panic and anxiety disorder.

Regular drinking of beet juice may have a profound effect in reduced risk and treatment of hypertension, a recent study suggested.

Hypertension is a condition of abnormal high blood pressure in the blood stream, considered as a major syndrome in increased risk of cardiovascular disease.

In a randomized crossover study of 24 subjects aged 25-68 years old divided into two groups with one group took raw beet juice (RBJ) for 2 weeks and the other group took cooked beet (CB). After 2 weeks of treatment, both groups had a washout for 2 weeks then switched to the alternate treatment, researchers after taking into account of other co founders found that both groups showed a remarkable in reduced blood pressure but significantly higher in beet juice subjects.

Dr. Asgary S1, the lead author said, " (Beside ameliorated blood pressure,) more improvement was observed in endothelial function and systemic inflammation with RBJ compared with CB".

Endothelial function plays an important role in ensured proper blood circulation in and out of the heart. Endothelial dysfunction is an abnormality of inner lining of blood vessels.

Systemic inflammation is an over production of inflammatory cytokines through immune response in activation of the innate immune system.

The study also addressed that beet juice and cooked beet attenuated hypertension may be resulted of a complex system involved  regulation of pro inflammatory and antioxidant expressions.

Furthermore, in the study of the effect of beetroot juice concentration in 24 overweight older participants given either the beetroot juice concentrate or blackcurrant juice group, with a 1-week post supplementation phase (week 4) for 3 weeks, scientists suggested that beetroot juice concentrate showed a strong inhibition of daily systolic BP after 3 weeks in compared to blackcurrant juice treatment group,

Systolic BP is the measurement of the maximum arterial pressure in stage of contraction of the left ventricle of the heart.

However,  the efficacy of beet juice in reduced BP can not be sustained after the interruption of the supplementation intake at 1 week followup.

Dr. Jajja A, the lead author said, "In overweight older subjects, beetroot juice concentrate supplementation was associated with beneficial effects on daily systolic BP".

Additionally, according to the joint review of the database included 85 older participants (age range: 55-76 years) of 2 trials with an open-label, parallel design in compared to  2 trials with a cross-over, double-blind design, researchers insisted that the efficacy of beet juice in reduced blood pressure may associate to age of subjects, as well as the expression of in-nocturnal mean.

The finding evidences demonstrated that  beet juice may be considered as a functional food in lowering high blood pressure.

Irritable bowel syndrome (IBS) is a disease affecting the large intestine, including symptoms of cramping, abdominal pain, bloating, gas, and diarrhea or constipation.

Yoga, the ancient technique  for harmonized external and internal body well beings, through breath control, meditation, bodily movement and gesture..... has been best known for people in Western world and some parts in Asia due to health benefits reported by various respectable institutes' research and supported by health advocates.

In a randomized controlled trials to compare the efficacy of yoga with usual care, nonpharmacologic, or pharmacologic interventions for patients with IBS through reviewing literature published on databases of MEDLINE/Pubmed, Scopus, the Cochrane Library, CAM-QUEST, CAMbase, and IndMED up to November 2015, after selected studies satisfied the criteria and guidelines, researcher filed the following results
1. Yoga participants expressed a significantly beneficial effect over conventional treatment in IBS,
2. Yoga intervention demonstrated a significantly decreased bowel symptoms, IBS severity, and anxiety.
3. Yoga group displayed an improved quality of life and physical functioning in compared to other treatment groups.
4. Yoga group also showed a little or no adverse effects in the selected studies

Conventional treatment of most common pharmacological approaches is to manage symptom of pain and digestive disorders due to significant side effects induced by prescribed medications.

Dr. Schumann D, the led authors said, " The findings of this systematic review suggest that yoga might be a feasible and safe adjunctive treatment for people with IBS". However, after taking into account of other con founders, he continued, " no recommendation can be made regarding yoga as a routine intervention for patients with IBS because of major flaws in study methods".

Further analysis into the study of total of 51 participants of adolescents (14-17 years).randomly assigned to a standardized 6-week twice weekly Iyengar yoga group-based program or a wait-list usual care control group, conducted by the University of California, scientists filed the results as follow
1. Yoga group showed a significant improvement of improved physical functioning
2. I compared to control, yoga participants also reported significantly improved IBS symptoms, global improvement, disability, psychological distress, sleep quality, and fatigue
3. 46% of YA reported a minimally clinically significant reduction in pain
4. At the end of the intervention, YA group in postlesson also reported a reduction of worst pain, constipation, and nausea.

 After taking into account of other confounders, Dr. Evans S, the led author indicated, "The findings suggest that a brief IY intervention is a feasible and safe adjunctive treatment for young people with IBS, leading to benefits in a number of IBS-specific and general functioning domains for YA" and "yoga interventions may be most fruitful when developmentally tailored".

Further studied illustration of 35 adult participants meeting ROME III criteria for IBS enrolled, 27 of the 35 participants (77%) completed treatment and pre- and post-treatment visits (89% women, 11% men; M (SD) age = 36 (13)), and 20 of the 27 (74%) randomly assigned to 16 biweekly group sessions of Iyengar yoga or a walking program and 6-month follow-up, found that
1. Yoga group showed an improvement from pre- to post-treatment when compared to control
2. Yoga group from pre- to post-treatment, also demonstrated significant decreases in IBS severity measures, visceral sensitivity and severity of somatic symptoms in compared to walking group.

At 6 months followup, researcher said, "overall GI symptoms for walking continued to significantly decline, while for yoga, GI symptoms rebounded toward baseline levels".

In fact, the efficacy of yoga in reduced symptoms of IBS can be attributed to the mediating effects of visceral sensitivity, increased psychospiritual resources, regulated autonomic nervous system responses and regulated hormonal stress response assessed via salivary cortisol, according to the University of California.

In simple English, yoga improved symptoms of treatment of patients with IBS through psychological symptoms, including improved mood , anxiety, depression measured scale, quality of life...... and physical symptoms, including getting rid of gas, reduced constipation, alleviated acid reflux......

The finding evidences suggested that yoga may be used as an adjunct intervention in assistance of standard therapy for treatment of irritable bowel syndrome (IBS).

Alternative Therapy: Yoga In Delay Aging Progression

Yoga may have a profound and positive effect in delay aging process, some renowned institute studies suggested.

According to the study to explore the impact of yoga and meditation based lifestyle intervention (YMLI) on cellular aging in 96 healthy individuals randomly assigned to a 12-weeks of YMLI, at the end of 12 weeks of lesson, YMLI group expressed significant improvements in both the cardinal biomarkers of cellular aging and the metabotrophic biomarkers influencing cellular aging in compared to baseline values.

The efficacy of the program in reduced cellular aging process was attributed to the activities in decreased production of ROS and pro inflammatory cytokins and hormone cortisol, and increased mean values of telomerase activity in regulated aging process and hormone β-endorphin in reduced stress and maintain homeostasis.

Dr. Madhuri Tolahunase, the led author, after taking into account of other co founders, said,"Lifestyle is an integrated entity, and an intervention, like YMLI, that has overall positive influence on our health appears most useful versus changing only one aspect at a time, as is seen by action of certain drugs. Yoga is holistic and a mind-body medicine and is more beneficial and advantageous than individual interventions like physical exercise, caloric restriction, and antioxidants".

Other researchers, in the study of yoga breathing in protected skin against aging, filed the following results
1. Yoga breathing reduced psychological aspects of stress and anxiety which have been found to associate to promoted aging
2. Yoga also expressed an impact in regulated  advanced glycation end products (AGE) which have recently been shown to play a role in tissue aging 
3. Comprehensive yoga program including breathing and meditative exercises may have a profound effect in enhanced gene expression involving oxidative stress, DNA damage, cell cycle control, aging and apoptosis and
4. Detoxification

In further analysis, Dr, Beri K the led author said, "(Contract to general belief)The interesting correlation of insulin regulation and glucose control can be translated to improvement and maybe reversal of the effects on AGE protein accumulation in the body tissue".

In support of the above differentiation, Dr. Brown RP at the Columbia University College of Physicians and Surgeons, launched an investigation of yoga breathing in delay aging process, suggested that
1. Yoga breathing (pranayama) can have a significant and positive effect in  bringing the mind to the present moment and reduce stress
2. Yoga breathing delayed the aging process through reduced expression of depression, anxiety, post-traumatic stress disorder, and for victims of mass disasters.
3. The practices also relieved many forms of suffering.

Finally, after taking account of other risk factors, researchers concluded that yoga breathing can affect longevity mechanisms.

Taking altogether, Yoga used alone or combined with meditation may  have a therapeutic effect in delay aging through regulating cellular and psychological expressions.
