Alternative Therapy: Yoga in Decreased Labor Pain and Improved Delivery Outcome

Yoga therapy may be considered as a secondary intervention combined with standard application in improved labor pain and delivery outcome and reduced risk of pregnancy complication.

1. Yoga group experienced less intensive pain, and at 2 h after the first and the second measurements in compared to control
2. Natural delivery was found more in the yoga intervention group in compared to more percentage of cesarean section in control
3. Yoga group also demonstrated decreased frequency of labor induction
4. And a shorter duration of the second and third stages of labor

With all information collected, Dr. Jahdi F, the led author said, "Yoga during pregnancy may contribute to a reduction pain of labor and improved adequacy of childbirth".

Other study in support to the above analysis, conducted by the Prince of Songkla University, in examined the effects of a yoga program involved six, 1-h sessions at prescribed weeks of gestation during pregnancy, on maternal comfort, labor pain, and birth outcomes. also suggested that
1. Yoga group expressed higher levels of maternal comfort during labor and 2h post-labor and
2. Less labor pain
3. Although both groups experienced same pain increased and maternal comfort at the beginning, yoga group showed a decreased pain and discomfort as labor progressed
4. Lastly, women in yoga class had a shorter duration of the first stage of labor and total time of labor.

More importantly, In a total of 335 women between 18 and 20 weeks of pregnancy, attending the antenatal clinic at Gunasheela Surgical and Maternity Hospital in Bangalore, India, enrolled in a prospective, matched, observational study with 169 women in the yoga group and 166 women in the control group, researchers emphasized that women participated in yoga lesson illustrated less complications such as preterm labor, isolated intrauterine growth retardation growth retardation and pregnancy-induced hypertension at the day of delivery in comparison to those in control.

Taking altogether, yoga intervention not only improved labor pain and delivery outcome but also reduced risk pregnant complications, therefore yoga may be recommended to pregnant women as a preventive measurements, particularly in high risk groups.

Bad news for pregnant women and coffee lovers, maternal caffeine intake may associate to increased risk of pregnancy loss, a renowned institute study suggested

Coffee, becoming a popular and social beverage all over the world, particular in the West, is a drink made from roast bean from the Coffea plant, native to tropical Africa and Madagascar.

Pregnancy lost is a natural death of an embryo or fetus before it is born, including spontaneous abortion, miscarry, stillbirth....

According to the joint study led by the National University of Singapore and National University Health System, additional intake of 1 cup of coffee in women who are regular coffee drinkers similar to 100 mg/d increment caffeine intake was associated with 7% higher risk of pregnancy loss.

In a total of 130 456 participants and 3429 cases in fourteen included studies, researchers found that women with
1. No or very low caffeine intake, have relative risk of pregnancy loss of 1.02
2. Moderate intake (150-349 mg/d) showed an increased relative risk of 1·16
3. High intake (350-699 mg/d) showed a relative risk of 1·40
4. Very high intake (≥ 700 mg/d) showed a relative risk of 1·72

The risks of pregnancy loss were increased linearly and correlatively to amounts of cup of coffee intake per day.

Furthermore, a case-control and cohort review study reported in English, the exposure of caffeine/coffee consumption during pregnancy, from selected 26 studies included (20 of caffeine and eight of coffee) suggested that the relative risk ratio of pregnancy loss due to caffeine consumption was 1.32

The study also found that risk of pregnancy loss increased substantially to 19% for every increased caffeine intake of 150 mg/day and every additional 2 cups of coffee intake, risk of pregnancy loss increased by 8%.

Although, there are many implications associated to the pregnancy loss such as ages of woman and man, greater body mass index,..... excessive coffee drinking may be one of the avoidable risk factor.

Taking all together, there is no doubt that higher intake of coffee during pregnancy induced high risk of pregnancy loss but according to the statistic, North American women seldomly drink more than 4 cups/day, so we may be in the safe zones.

The study also indicated that decaffeinated coffee and caffeine showed no statistically significantly associated with ovarian cancer incidence.

Other, in the evaluated enzyme CYP1A2 in the metabolism of coffee from the data collected from a preliminary investigation conducted in Hawaii of 164 epithelial ovarian cancer cases and 194 controls, women with CYP1A2 genotype modifies, the results showed that coffee consumption in these population expressed a substantially increased risk of ovarian cancer in compared to women who did not drink regular coffee.

Furthermore, Women with any CYP1A2 C allele and women with the A/A genotype were associated to similar risk of ovarian cancer (OR = 1.1, 95% CI = 0.7-1.7) but the increased risk of ovarian cancer was stronger among women with the A/A genotype than among women with any C allele.

Dr. Goodman MT, the lead author said., "preliminary data suggest a modest positive association of caffeine and coffeeconsumption with the OR for ovarian cancer that may be modified by CYP1A2 genotype and exposures, such as cruciferous vegetable consumption, that influence CYP1A2 expression".

However, according to the Information on coffee consumption collected from 91,767 women at baseline in the Norwegian Women and Cancer Study, coffee intake was found to reduced risk of various types of cancer, including ovarian caner, with no statistic different in women who drank more than 3 and up to 7 cups/day, and women who drank ≤1 cup/day.

Taking together, intake of coffee regularly with less than 5 cups per day may be associated to ameliorated risk of ovarian cancer incidence, but women with  enzyme CYP1A2  modified and  C allele and post menopause women should consult with their doctor before applying.

Interestingly, in a randomized, controlled clinical trial to evaluate MBSR as a treatment for chronic insomnia of 18 adults completed an 8-week mindfulness-based stress reduction (MBSR) program, researchers at the University of Minnesota filed the report as follow
1. Yoga, mindfulness-based stress reduction (MBSR) program intervention do not induce more sleeping time, but sleeping better, waking more refreshed, feeling less distressed,.......

According to the University of Nebraska Medical Center, coffee intake in the experiment model showed no significant differences in the frequency or severity of dyspepsia between the coffee brewed with coffee beans processed using conduction roasting in compared to other types of roasted coffee.

4. The prevalence of coffee induction of dyspeptic symptoms was much more common in non ulcer dyspepsia patients (53%) than in controls (22%).

Dr. Elta GH, the lead researchers said, "patients with non ulcer dyspepsia,....... were more likely to experience dyspeptic symptoms after coffee ingestion".

Taking together, coffee regardless to types does not exhibit risk but elevated symptoms of dyspepsia. According to the Royal Adelaide Hospital, identifying specific foods as triggers of the symptoms,and make behavioural adjustments in daily diet and life style change may improve symptoms, including reduced intake of implicated foods such as milk and dairy products, citrus fruits, spicy foods, coffee and alcohol.

Food Therapy: Bean Fibers In Reduced Risk of Abnormal Blood Glucose

Epidemiological studies suggested that bean fiber may have a potential used as therapeutically functional food in reduced risk of hyperglycemia.

Hyperglycemic is a condition characterized by abnormal high blood sugar in the blood stream.

According to the University of Wisconsin-Madison,  dietary fiber intake daily and regularly not only lowers blood cholesterol levels but also normalizes blood glucose and insulin levels, thus reducing risk and treatment of cardiovascular disease and type 2 diabetes.

Unfortunately, recommended intakes, 20-35 g/day for healthy adults and age plus 5 g/day for children, are not being met, according to statistic.

In a randomized, single blind, crossover design study (food intake: n=22; glycemia: n=10) with 5 liquid meals distributing similar amounts of macronutrients (C - control shake, OB - oat bran shake, F - flaxseed shake, WB - white bean extract shake, and UB - unripe banana flour shake) consumed in five non-consecutive days, returned questionnaire from participants after 15, 30, 45, 60, 90 and 120 minutes ingestion of each shake suggested that
1. In compared to control, intake of white bean extract shake showed a significantly increased fiber component.
2. Bean extract exhibited a strong glycemic control through expression of an incremental areas under the curves (iAUC).

Additionally, in the study of children completed a trial of 4 different breakfasts, given in random order, with 3 diets differed in fiber content and the fourth diet contained soya beans as part (38%) of the dietary fiber source  were asked to exercise vigorously for an hour each morning and to rest for an hour, with no different mean diet contributed in the morning, after taking account of other risk factors, blood samples drawn from children reveal the following results.
1. There were no difference of blood glucose levels on the high-fiber diet in compared to those on the medium-fiber diet
2. Children fed with bean diet scored a significantly lowest mean blood glucose level and
3. The effect of glycemic control of blood glucose of bean diet was last into the hour before lunch.
Even with the better control of blood glucose, the bean diet was found to be unacceptable diet among children, in compared to the liked of the high- and medium-fibre diets, which were as popular as the low-fiber diet.

Taking altogether, there is no doubt that bean fibers may have a positive effect in attenuated risk of abnormal high blood glucsoe, a major risk factors in increased risk of type 2 diabetes and cardiovascular disease.
