1. Emergency treatment
An acute subdural hematoma (SDH) is a rapidly clotting blood collection(548) below the inner layer of the dura but external to the brain and arachnoid membrane(549). Two further stages, subacute and chronic, may develop with untreated acute subdural hematoma (SDH)(549). There is always important to maintain survival of the patient with acute subdural hematomas(550)(551) because of its an unfavorable outcome in the majority of cases(551). Emergency treatment is necessary to reduce pressure and allow blood to drain by drilling a small hole in the skull and  inserting a temporary small catheter through a hole drilled through the skull and sucking out the hematoma(552)(564).  Although hematoma resolution has been reported, it cannot be reliably predicted, and no medical therapy has been shown to be effective in expediting the resolution of acute or chronic subdural hematomas(552)(553).

2. Medication
Inn case of chronic subdural hematomas, Mannitol may be used to reduce intracranial pressure (ICP)(554)(555) as it produced a significant reduction in ICP and improved cerebral perfusion pressure(556).
a. Corticosteroids for brain oedema
Methylprednisolone is a synthetic glucocorticoid or corticosteroid drug(557). Researchers found that Methylprednisolone can effectively reduce myelin changes(559) accompanying brain oedema(558) induced by blood-brain barrier opening with an osmotic insult(559).

b. Anticonvulsants for patient with seizures
In some cases, patients with chronic subdural haematoma may be treated with anticonvulsants for seizures prevention(560). The medicine has shown to reduce risk of seizures(562) to none and 1.8%  in 73 patients given prophylactic antiepileptic drug treatment in Tokyo Medical and Dental University study(561) and Beilinson Medical Center(562) studies, respectively.

c.  Rifampicin for bacterial infection
Rifampicin is a naturally made, non-peptide antibiotic(563). It is bactericidal, killing agent  by disabling the protein expression system universally conserved by all bacterial infection(567), but it can induce thrombocytopenia(565)(566) in acute subdural hematoma treatment.

3. Surgery
Large or symptomatic hematomas require a craniotomy, as a bone flap is temporarily removed from the skull to access the brain for removal of blood clot with suction or irrigation(568). According University of Cambridge, Cambridge, the use of a drain after burr-hole drainage of chronic subdural haematoma and minimized the incidence of significant recollection(570) is safe and associated with reduced recurrence and mortality at 6 months(569).

B. Multi-infarct dementia
Also known as vascular dementia, is the second most common form of dementia after Alzheimer's disease in older adults between ages of 60 and 75(571), caused by different mechanisms all resulting in vascular lesions in the brain(572)(573) with prevalence of major depression, depressed mood/anhedonia, and subjective and neurovegetative symptoms of depression(574).

II. Treatments of Multi-infarct dementia

C.3. Dementia associated to Creutzfeldt-Jakob disease
People who have eaten contaminated beef in a prolonged period of times may be infected by infectious agent prion(514) without even knowing it. Creutzfeldt-Jakob disease is a quickly progressing and fatal disease(513) inducing dementia(515), especially in elder(516) and degeneration of skeletal muscle, peripheral nerves(517) linked to mutations in the PrP gene(518). CJD is characterized by rapidly progressive dementia(513)(519). Initially, individuals experience  of epilepsy seizure(519), problems with muscular coordination(522); cognitive change (loss of motor planning, loss of motor functioning's, inability to speak)(519), such as  impaired memory(521), loss of functional independence(523) and impaired vision(520). People with the disease also may experience insomnia(524)(525), depression(526)(527), or unusual sensations(522).

II. Treatments of Creutzfeldt-Jakob disease (CJD)
There is no treatment that can cure or control CJD. The available treatments are to relieve the symptoms and may help slow the disease.
1. Interleukins
a. Interleukins is defined as any group of naturally occurring proteins that mediate communication between cells(528), produced by while blood cells. The set of interleukins stimulated by a specific infectious agent in determined cells in responding to the infection and influences(528) through its modulated inflammation and immune response.(529).
According to University Hospital Göttingen, interleukin 10 levels, inflammatory cytokines(530) were significantly elevated in the cerebrospinal fluid of CJD, dementia, motoneuron disease patients through it inflammatory cytokines(529). Cyclooxygenase-2 (COX-2)(532) and prostaglandins (PGs)(533) are the most conventional medicine used to treat neurotoxiticy in acute conditions, including in inflammatory chronic diseases, such as Creutzfeldt-Jakob disease (CJD) and Alzheimer's disease (AD)(531).

b.1.1 Insomnia,
b.1.2 Abdominal pain,
b.1.3. Flatulence (gas),
b.1.4. Headache ,
b.1.5.  Nausea and diarrhea.

b.2. Dizziness
b.2.2. Fainting
b.2.3. Irregular heartbeat or pulse•
b.2.4. Slow heartbeat

2. Other medication
2.1.  Quinacrine
a. Quinacrine used for treatment of giardiasis caused by Giardia lamblia(535) may be a potential medicine for treatment ofCreutzfeldt-Jakob disease(CJD)(536)(537), according to a report in The Mail on Sunday, on Sunday 12 August 2001, entitled "Briton 'cured' in CJD drug trial"(534).
Although Quinacrine at a dose of 300 mg per day was reasonably tolerated, it did not induce significantly affect in course of prion diseases(537), including Creutzfeldt-Jakob disease (CJD(538)(539).

2.2. Gamma-aminobutyric acid, dopamine and serotonin
 a. Other medication used to control aggressive and uncontrolled behavior, such as gamma-aminobutyric acid(541)(542)(543) with functions of inhibitory neurotransmitter in the mammalian central nervous system(540), (541)(543) dopamine with functions as a neurotransmitter and serotonin(542)(543) also functions as a neurotransmitter may be helpful.

Scientific Advisory Board, Osteoporosis Society of Canada, Screening and diagnostic methods: risk-factor assessment, clinical evaluation, measurement of bone mineral density, laboratory investigations.

2. Dual energy X-ray absorptiometry (DXA)
Dual energy X-ray absorptiometry (DXA) is one most common test to measure the total bone density of including spine, hip, wrist etc. with accurate result. 


3. Quantitative Ultrasound and computed tomography (QCT)
 Quantitative ultrasound (QUS) is a
portable and accurate technology used to  evaluate bone density at the lumbar spine and hip without the use of ionizing radiation.
Computed Tomography (CT) scanner.a technology for measuring properties of bone at peripheral skeletal sites(23).
Dual-energy x-ray absorptiometry (DXA) and quantitative computed tomography (QCT), which are now the standard methods for assessing osteoporosis severity and treatment efficacy(24), but in cross-sectional study of males with glucocorticoid-induced osteoporosis (GIO, quantitative computed tomography (QCT), High-resolution quantitative computed tomography (HRQCT)-based  were more superior to DXA in discriminating between patients of differing prevalent vertebral fracture status(25).

Osteoarthritis (OA) is a widespread degenerative disease of skeletal joints and often associated with senescence in vertebrates due to excessive or abnormal mechanical loading of weight-bearing joints,  arising from heavy long-term use or specific injuries(6).

2. Nutrient deficiency
Poor nutritional conditions in early life are linked to greater prevalence of OA due
the gradual deterioration of function(9).

Cartilage is a flexible connective tissue which cushions the ends of bones in your joints to allow the joints to move smoothly. If the cartilage becomes rough  or wears down due to aging or damage, it can cause pain as a result of  bone in the joint rubbing against another bone.

1. Young Age at Diagnosis, Male Sex, and Decreased Lean Mass

According to Korea Cancer Center Hospital, Seoul,
a. Diagnosed before attainment of puberty, were showed to have a higher prevalence of osteoporosis
b.  If you are males, you are at increased risk of osteopenia or osteoporosis than females
c.  Regional lean mass was significantly associated to the reduce risk of affecting the limbs

2. Adult growth hormone replacement

Adult growth hormone replacement, were identified to associated to increased risk of osteoporosis and osteopenia, according to the study by Seoul National University College of Medicine(13).

3. Aging

Risk of  osteoporosis increases with age.
Bone loss occurs during the normal aging process. In women, natural menopause also effect the additional bone losses,  according to the study by Department of Medicine, College of Physicians and Surgeons, Columbia University(14).


4. Chlamydia pneumoniae
  Individual presented of Chlamydia pneumoniae DNA  are alsp associated to both in osteoporotic bone tissue(15).


5. Race
Lactose maldigestion showed a greater affect on low bone density. Extensive lactose maldigestion among Hispanic-American and Asian-American populations may elevate the risk for osteoporosis(16).

According to the reported physician-diagnosed osteoporosis and family history in a representative sample of U.S., family history is associated to a significant, independent risk factor for osteoporosis in U.S. women aged>or=35 years(17).


7. Body size
  Large body size is associated to the risk of the development of osteoporosis and a salutary effect on BMD in both blacks and whites, in a study of
three groups of postmenopausal women: 104 healthy black women, 45 healthy white women, and 52 osteoporotic white (18).
 
8.  Diet and lifestyle

BMD was higher in habits of alcohol drinking, green tea drinking, and physical activity and lower in those with the habits of smoking and cheese consumption, in a study of total of 632 women age > or =60 years(19).

9. Heavy alcohol intake or alcoholism
Heavy alcohol intake or alcoholism, however, frequently disrupts calcium and bone homeostasis, leading to reduce bone mineral density and increase the incidence of fragility fracture, according to the study by Department of Endocrinology and Metabolism, Saitama Medical School(20).


10. Smoking and lower serum IGF-I levels
  A lower BMI is found in patient who are current smoking history and lower serum IGF-I levels in middle-aged Korean men(21).


11. Other risk factors
The frequency of decreased bone mineral density, low vitamin and calcium diet content and insufficiency with vitamins are found among patients suffering from chronic diseases (of cardiovascular system, gastrointestinal tract, osteopenia and osteoporosis)(22).

Wonderful for luncheon. Creamy and light.

Lime flavored gelatin 1 x 3 oz. 1 x 85 g
Boiling water 1 cup 225 ml

Pear juice 1 cup 225 ml
Cream cheese 4 oz. 125 g
Small marshmallows(or 12 large. cut up) 100
Canned pear halves, diced 8
Whipping cream( or 1 envelope topping) 1 cup 250 ml

In a large bowl put gelatin and boiling water. Stir to dissolve.

Add pear juice. Using wire cheese cutter, cut cheese and add in small chunks. Stir in marshmallows. Chill, Stirring now and then until mixture begins to thicken.

Fold in diced pears. Whip cream until stiff. Fold into mixture. Pour into mold or serving bowl. For standing in warm room, better to use a serving bowl since this is not too firm a salad. Serves 12.

Osteoporosis is condition of thinning of bone and bone tissues as a result of  the loss of bone density over a long period of time. 

I. Symptoms
1. Lower back pain, as a result of fractured or collapsed vertebra

People with Osteoporosis are susceptible to lower back pain than those without osteoporosis, according to a cross-sectional study, conducted from October 2002 to March 2003 to gather data from 24,435 adults aged 20 years and older selected randomly from Taiwan’s general population. The Osteoporosis rate of frequent low back pain in association with osteoporosis in men and women were 5.77 and 3.49 respectively(1).

2. Loss of height over time

fractures in the spine, hip and wrist), among men with osteoporosis, according to a multicenter, double-blind, placebo-controlled trial of randomly assigned 1199 men with primary or hypogonadism-associated osteoporosis who were 50 to 85 years of age(5).

5. Neck and low back pain
  Neck and low back pain,
frequent in female are prevalent and highly associated in patients with headache and osteoporosis, according to 1-year prevalence of neck pain and low back pain in the Spanish population(6).

6.  Depression

Spine BMD was negative correlations between anxiety, stress, but depression was found to have a unique significant contribution to the explained variance in right and left hip BMD(7).

7. Other symptoms
Women with osteoporosis  re more likely to experience certain
psychosomatic, gastrointestinal and swelling and vasomotor symptoms, especially depressive symptom if compared to women in generally good health(8).

Cream green, somewhat tart and so easy.

Lime flavored gelatin 1 x 3 oz. 1 x 85 g
Boiling water 1 cup 225 ml
Crushed pineapple and juice 14 oz. 398 ml

Sour cream 1 cup 250 ml

Put gelatin in bowel. Stir in boiling water dissolve. Add pineapple with juice. Stir. Chill until syrupy.

Stir sour cream into thickened gelatin. Pour into mold or serving bowl. Chill. Serves 8.

A good, fluffy mint green color salad- a disguise for vegetable.

Lime flavored gelatin 1 x 3 oz. 1 x 85 g
Boiling water 1 cup 225 ml
Salt 1/4 tsp. 225 ml
Granulated sugar 1 tbsp. 1 ml
Cold water 1/2 cup 125 ml
Salad dressing (or mayonnaise) 1/2 cup 125 ml

Grated cabbage 2 cup 500 ml
Grated carrot 1/2 cup 125 ml
chopped celery 1/2 cup 125 ml
Copped peeled cucumber 1/4 cup 50 ml
Grated onion ( or 1 tsp. , 5 ml, onion flakes) 1 tbsp. 15 ml

Dissolve gelatin in boiling water in bowl. Stir in salt and sugar. Add cold water.
Whisk or beat in salad dressing to combine. Chill until slightly thickened. Beat with rotary beater until fluffy.

Fold in cabbage, carrot, celery, cucumber and onion. Pour into mold or serving bowl. Chill. Serves 8

Note: Gelatin may be completely set then  put in a blender to fluff before adding vegetables. It will make it nice and airy but it won't increase the volume as much when using a rotary beater.

Glaucoma is a medical condition of an eye disease caused by induced retinal ganglion cell types alterations before detectable dendritic loss. If untreated, the disease can lead to permanently vision impair and blindness.
II. Causes and Risk Factors
A. Causes
1. Genes
a. Certain genes mutation, such as Myocilin(MYOC), Cytochrome P450, family 1, subfamily B, polypeptide 1(CYP1B1), and Forkhead box C1(FOXC1)
In primary Glaucoma, Matrix metalloproteinases (MMPs) play an important role on cell behaviors such as cell proliferation, migration (adhesion/dispersion), differentiation, angiogenesis, apoptosis, and host defense(23), defect of MMPs (MMP1 rs1799750 (-1607 1G/2G) and MMP9 rs17576 polymorphisms(25)) have been shown to associated with gender-dependent risk factors for developing primary open-angle glaucoma (POAG) and primary angle closure glaucoma (PACG), respectively(24)(26).
According to studies,
a.1. Although it is rare, genetic mutation of genes cytochrome P450, family 1, subfamily B, polypeptide 1(CYP1B1), myocilin (MYOC), optineurin(OPTN), and Opticin(OPTC) is associated to increased risk in Indian population(28).;
a.2. Mutation of  genes myocilin (MYOC) and optineurin (OPTN) sequence variations is associated to  risk factor in Spanish(29) and
a.3. Mutation of myocilin (MYOC),optineurin (OPTN), WD repeat domain 36 (WDR36), and apolipoprotein E (APOE) genes are increased risk of primary open-angle glaucoma (POAG) in northern Chinese(30).
a.4. Although other gene mutations, such as Latent Transforming Growth Factor Beta Binding Protein(LTBP2) has been found in north Indian patients with PCG, it is not involved in the pathogenesis of primary congenital glaucoma(27).


b. Connective tissue growth factor (CTGF)
The most critical risk factor for optic nerve damage in cases of primary open-angle glaucoma (POAG)(31) is an increased intraocular pressure (IOP) due to alternation of tissues in the eye located around the base of the cornea(32). Actin, a critical player in many cellular functions, including cell motility and the maintenance of cell shape, etc.(33), disruption or taken over actin cytoskeleton by numerous pathogens(34), may induce a resistance to aqueous humor outflow in the trabecular meshwork (TM)(36), an area of tissue is responsible for draining the aqueous humor from the eye via the anterior chamber causes of optic nerve damage involved connective tissue growth factor (CTGF), found in higher amounts in the aqueous humor of patients with POAG(35).

c. Alterations of serine protease
Angle-closure glaucoma (ACG) is a subset of glaucoma affecting 16 million people. According to the study by the The Jackson Laboratory, alterations of  serine protease, a family of enzymes that cut certain peptide bonds in other proteins with functions, especially in digestion, blood clotting, and the complement system(37) may induce a wide range of  ocular conditions, including reduced ocular size and ACG(38)(39) and high hyperopia(39).

2. Iactive congenital syphilitic interstitial keratitis
Iterstitial keratitis(IK), a condition of corneal scarring caused by chronic inflammation of corneal stroma(40) has been found to associate to the development of angle-closure glaucoma in the absence of corneal opacity in patinets(41).

3. Anatomical disorder(42)
Chronic angle-closure glaucoma, the commonest form of glaucoma in Sino Mongaloid populations can lead to permanent closure of part of the filtration angle in the anterior segment(42)(43) of the eye, leading to conditions of recurrent pupillary block and “creeping” angle closure(44).

4. Atypical ocular ischaemia
 Low intraocular pressure in untreated glaucoma(45) can lead to ciliary shut down, retinal detachment, post filtering surgery and ocular ischaemia, according to Glaucoma Services, LV Prasad Eye Institute(46).

5. Other causes
According to the examination of post operation patients with glaucoma in the first 275 consecutive eyes, other causes include
a. Exacerbation of a pre-existing glaucoma
b. IOP elevation induced mechanical angle closure, due to impaired ocular blood flow(48)
c. Steroid response(49)(50)
d. Peripheral anterior synechiae(51)(52)
e. Without detectable cause (47).

Makes a clear vegetable packed salad.

Lime flavored gelatin (jelly powder) 1 x 3 oz. 1 x 85 g
Boiling water 3/4 cup 175 ml
Salt 1/4 tsp. 1 ml
Cold water 3/4 cup 175 ml

Grated cabbage 1 cup 250 ml
Grated carrot 1/2 cup 125 ml
Diced celery 1/2 cup 125 ml

Dissolve gelatin in boiling water in bowl. Stir salt. Add col water. Chill until slightly thickened.

Add cabbage, carrot and celery to thickened gelatin. Fold in. Pour into mold. Chill Garnish with sliced cucumber or other vegetables. serves 8.

(Preview) Most Common Diseases of Ages of 50 Plus - Glaucoma: The Types

Glaucoma is a medical condition of an eye disease caused by induced retinal ganglion cell types
alterations before detectable dendritic loss. If untreated, the disease can lead to permanently vision impair and blindness.

I. Types of glaucoma(a)
There are several types of glaucoma. The two main types are (Chronically)open-angle(2)(3) and (Acutely)angle-closure(4)(5) both caused by either increase of intraocular pressure (IOP)(1), or pressure inside the eye(1).

A. Open-Angle Glaucoma affecting about 2.2 million Americans(6) and over 90% in all glaucoma cases(7), is the most common form of glaucoma, according to the 2000 statistic, with rates increased with ages(7).
Due to gradually progressive visual field loss, and optic nerve changes of the disease(9), patients may not experience any symptoms in the early stage of the diseases(10) depending to IOP high-tension and larger IOP fluctuations in normal-tension of OAG patients(8). Lacerating injury of cornea with iris prolapse in the eye if left untreated, may also induce open angle glaucoma(11).

A.1. Open-Angle Glaucoma caused by the slow progression of clogging of the drainage canals(13)  induced primary open-angle glaucoma (POAG) and pseudoexfoliative glaucoma (PEXG), due to increased intraocular pressure (IOP)(12).

A.2. Open-Angle Glaucoma is a result of the area  between the iris and cornea (The angle) widen(11).
The untreated rate of progression of patients with normal-tension glaucoma (NTG), and pseudoexfoliation glaucoma (PEXG) corresponded to advancing from normal visual function to blindness in approximately 70 years(14).

B. Angle-Closure Glaucoma
Angle-closure glaucoma is a less common form of acute glaucoma. Patients with angle-closure glaucoma may be experience the symptoms of sudden ocular pain, seeing halos around lights, red eye, very high intraocular pressure, nausea and vomiting, suddenly decreased vision, and a fixed, mid-dilated pupil(15), etc.
1. Caused by Sudden rise in intraocular pressure
Psychotropic agents(16)(17)(18) have been found to be frequently associated with ocular adverse effects(21) in inducing  angle-closure glaucoma, including pseudoexfoliation glaucoma caused by intraocular pressure (IOP) increased within hours after carbamazepine and gabapentin intake(19).

2. Caused by a closed or narrow angle between the iris and cornea(20)
Closed or narrow angle between the iris and cornea may be associated to early stage of acute angle-closure glaucoma(22).
