Coffee, has become a popular and social beverage all over the world, particular in the West, is a drink made from roast bean from the Coffea plant, native to tropical Africa and Madagascar.

Intake of coffee in adulthood may increase risk of autoimmune diabetes or type 1 diabetes, a study of Institute of Environmental Medicine, Karolinska Institutet suggested.

In a population-based case-control study with incident cases of adult onset (≥ 35 years) diabetes, including 245 cases of latent autoimmune diabetes in adults (glutamic acid decarboxylase antibody positive), 759 cases of Type 2 diabetes (glutamic acid decarboxylase antibody negative), together with 990 control subjects without diabetes, randomly selected from the population, after inspection of the returned questionnaire, researchers found that coffee intake may be associated with an increased risk of high glutamic acid decarboxylase antibody latent autoimmune diabetes in adults.

The relative odd ratio of these population was 1.11 in compared to non drinkers.
The risk of disease development increased by 15.2% (P = 0.0268) for every additional cup intake.

Dr. Löfvenborg JE, the lead author said, " coffee may be associated with development of autoimmunity and possibly an increased risk of more Type 1-like latent autoimmune diabetes in adults".

More importantly, in the review of the association between coffee intake, its influence on the immune system and the insurgence of the most relevant autoimmune diseases conducted by the joint study lead by the Tel-Aviv University coffee intake showed a selectively both in increased(rheumatoid arthritis (RA) and type 1 diabetes mellitus (T1DM)) and decreased(multiple sclerosis, primary sclerosing cholangitis, and ulcerative colitis) risk of some auto immune diseases.

Particularly in type 1 diabetes, Dr. Sharif K, the lead author said, "Coffee intake led to a decrease in insulin sensitivity in T1DM" and " coffee consumption was shown to influence disease course and management options".

Remarkably, in the study of some diabetic children younger than 15 years, and diagnosed from September 1986 to the end of April 1989, with 600 newly diagnosed diabetic children and 536 randomly selected population-based children, researchers postulated that risk for type 1 diabetes were increased in the children who consumed at least 2 cups of coffee daily in compared to other groups.even after adjusting to other odd factors.

The finding evidences suggested that coffee intake daily and regular may have significant impact in increased development of type 1 diabetes in children and adults population regardless amount of cups intake. therefore, people with family history of type 1 diabetes should reduced intake of less than 2 cups of coffee per day.

Food Therapy: Coffee Increases Risk of GERD in Geographical or Racial Differences?

Epidemiological studies linking coffee intake in risk of Gastro-esophageal reflux disease have been inconsistent and contradictory.

Coffee, a popular and social beverage all over the world, particular in the West, is a drink made from roasted bean from the Coffea plant, native to tropical Africa and Madagascar.

Gastro-esophageal reflux disease is a digestive disorder, affected ring of muscle between the esophagus and stomach, inducing symptoms of heartburn or acid indigestion.

Pathophysiological changes in esophageal functions that occur with aging may, at least in part is responsible for the high prevalence of Gastro-esophageal reflux disease (GERD).

In the review of online literature published database from Med, EMBASE, and Cochrane Library up to December 2012, researchers at the Seoul National University Hospital filed the following results
1. A meta-analysis showed that intake of coffee exerted no significantly associated risk of GERD.
2. Only the endoscopy group showed a significantly higher odds ratio risk induced by coffee in compared to other subgroups.

But after accounting to other risk factors, such as amount of coffee intake, quality of study, and assessment of exposure researchers found that coffee intake do not associate to increased risk of GERD.

Other, in the study to investigate the association of habits and social conditions reported lead to reflux in the employees of hospital, researchers indicated that even with the the prevalence rate of 21.7% of GERD incidence in this group of participants, intake of coffee daily and regularly showed no effect in increased risk GERD, regardless to amount of coffee intake per day.

Contrast to the above studies,  the Korean prospective study of recruited subjects with GERD visited a health promotion center for upper gastrointestinal cancer surveillance at Hallym Medical Center (five institutions) between January 2008 and February 2009, researchers reported that
1. Coffee intake showed a significant correlation to independent risk of GERD in young and adults groups
2. and Coffee intake was considered as a major risk factor in elderly group.

Dr. Park CH, the lead author said, "The risk factors for RE according to age group were found to differ. In elderly group, Helicobacter pylori infection was not a significant protective factor contrary to young and adult groups.

Taking altogether, Coffee intake showed a conflict result in risk of GERD, probably due to geographic difference as Asian population are susceptible to risk of the diseases in compared to population in the West. Further studies are necessary.

According of the joint study lead by the Biospectrum Life Science Institute, in vitro, ex vivo, and in vivo, Andrographis paniculata extract (APE) exhibited a significant anti aging activity through ameliorate the process of loss of human epidermal stem cells (EpSCs).

Truly, the epidermal stem cells (EpSCs). treated with Andrographis paniculata extract (APE) exerted an anti aging effect by increased cells quantity significantly in the G2/M and S stages in cell division in a dose-dependent manner. 

The study also found that APE-treated EpSCs enhanced the Type 1 collagen production in normal human fibroblasts (NHFs).and improved skin hydration, dermal density, wrinkling, and sagging significantly.

In UV-Induced Skin Photoaging Andrographolide sodium bisulfate (ASB), a water-soluble form made from andrographolide increased the function of antioxidants in attenuated oxidative stress and inflammation responsible for ultraviolet (UV) irradiation causing skin damage and premature aging.
Dr. the lead anthor in the study of ultraviolet (UV) induced skin aging said, "topical administration of ASB suppressed the UV-induced skin thickness, elasticity, wrinkles, and water content, while ASB, especially at dose of 3.6 mg/mouse, increased the skin collagen content by about 53.17%, decreased the epidermal thickness by about 41.38%, and prevented the UV-induced disruption of collagen fibers and elastic fibers"

Furthermore, the study also proposed, the anti aging effect of the herbal Andrographis may be attributed  to anti enzymes in inhibited free radicals induced of oxidative stress and reduced the expression of pro inflammatory cytokins.

Taking together, Andrographis paniculata expressed significantly anti-skin aging effects through various mechanisms, but intake of large amount should be taken with care.

Epidemiological studies do not agreed that coffee regular intake is associated to risk of endometriosis, according to the IRCSS Fondazione Ca' Granda.

In a meta-analysis of epidemiological studies published up to January 2013 to analyze the relative risks (RR) of endometriosis for any, high and low versus no coffee/caffeine consumption, researchers filed the following reports
1. A total of six case-control and two cohort studies, including a total of 1,407 women with endometriosis were found in this review
2. The relative ratio were 1.26 and 1.13 for coffee and caffeine consumed participants respectively, in compared to non-consumption group
3. The overall relative ratio was 1.18 in coffee consumption group after taking into account of numbers of cup of coffee intake and other risk factors
4. Risk of endometrosis increased reduced to 1.09 in compared to high and low caffeine consumption as compared to no consumption, respectively.

Dr. Chiaffarino F, based on the information of this review said, "The present meta-analysis provided no evidence for an association between coffee/caffeine consumption and the risk of endometriosis. Coffee/caffeine consumption, as currently used in diet, does not carry a health risk".

Other, in the examine data from two case-control studies conducted in Northern Italy between 1984 and 1999 with 504 women aged < 65 years (median age 33 years, range 20-65) with a laparoscopically confirmed diagnosis of endometriosis, admitted to a network of obstetrics and gynaecology departments in Milan, Brescia and Pavia and 504 healthy women acted as control, researchers at the Università di Milano after adjusting to other risk factors suggested that there may be a link between diet and risk of endometriosis but coffee intake do not form significant risk of endometriosis incidence.

Contrast to the above studies, according to the cross-sectional study included 28,822 women, 1,228 women with endometriosis and the self-reported diagnosis was confirmed by medical records conducted by the joint institutes lead by the Department of Medical Epidemiology and Biostatistics, Karolinska Institute, coffee intake was associated to risk of development of endometriosis.

The study also indicated that even coffee showed a positive association to risk of endometriosis, but future studies are needed to explore the significance of other factors such as age at menarche, level of education, body mass index (BMI), parity, oral contraceptive (OC) use, infertility, smoking, and alcohol intake in the diagnosis of endometriosis and understanding of its etiology.

Taking altogether, although there are conflict information related to coffee intake and risk of endometriosis, but women with family history of ednometriosis are recommended to reduce intake of coffee and coffee caffeine and consult with their doctors before applying high amount of coffee daily.

Additionally, dichloromethane fraction of the methanolic extract.of Andrographis paniculata also expressed an impressive inhibition against testing human cancer and immune cells.
At low concentration, the dichloromethane extract enhanced immune function in attenuated the proliferation of HT-29 (colon cancer) cells and human peripheral blood lymphocytes (HPBLs).

Furthermore, the study also found, andrographolide, another major compound in Andrographis paniculata exhibited immune ability to suppress irregular cell growth and cancer activity on diverse cancer cells in different types of human cancers.

Taking altogether, there is no doubt, that Andrographis consisted a strong immunomodulatory activity in regulated immune respond against procession of irregular cell growth and over reaction of foreign harmful substances, but intake of large amount should be taken with care.
