Osteoarthritis

Osteoporosis is condition of thinning of bone and bone tissues as a result of  the loss of bone density over a long period of time. 

I. Symptoms

4. Chlamydia pneumoniae

5. Race

7. Body size
  Large body size is associated to the risk of the development of osteoporosis and a salutary effect on BMD in both blacks and whites, in a study of
three groups of postmenopausal women: 104 healthy black women, 45 healthy white women, and 52 osteoporotic white (18).
 
8.  Diet and lifestyle

BMD was higher in habits of alcohol drinking, green tea drinking, and physical activity and lower in those with the habits of smoking and cheese consumption, in a study of total of 632 women age > or =60 years(19).

9. Heavy alcohol intake or alcoholism
Heavy alcohol intake or alcoholism, however, frequently disrupts calcium and bone homeostasis, leading to reduce bone mineral density and increase the incidence of fragility fracture, according to the study by Department of Endocrinology and Metabolism, Saitama Medical School(20).


10. Smoking and lower serum IGF-I levels
  A lower BMI is found in patient who are current smoking history and lower serum IGF-I levels in middle-aged Korean men(21).


11. Other risk factors
The frequency of decreased bone mineral density, low vitamin and calcium diet content and insufficiency with vitamins are found among patients suffering from chronic diseases (of cardiovascular system, gastrointestinal tract, osteopenia and osteoporosis)(22).

III. Diagnosis According to the Clinical practice guidelines for the diagnosis and management of osteoporosis. Scientific Advisory Board, Osteoporosis Society of Canada, Screening and diagnostic methods: risk-factor assessment, clinical evaluation, measurement of bone mineral density, laboratory investigations.

1. Blood and urinary tests
The aim of the tests is to check for the bone metabolism and the progression of bone (loss) diseases.

2. Dual energy X-ray absorptiometry (DXA)
Dual energy X-ray absorptiometry (DXA) is one most common test to measure the total bone density of including spine, hip, wrist etc. with accurate result. 


3. Quantitative Ultrasound and computed tomography (QCT)
 Quantitative ultrasound (QUS) is a
portable and accurate technology used to  evaluate bone density at the lumbar spine and hip without the use of ionizing radiation.
Computed Tomography (CT) scanner.a technology for measuring properties of bone at peripheral skeletal sites(23).
Dual-energy x-ray absorptiometry (DXA) and quantitative computed tomography (QCT), which are now the standard methods for assessing osteoporosis severity and treatment efficacy(24), but in cross-sectional study of males with glucocorticoid-induced osteoporosis (GIO, quantitative computed tomography (QCT), High-resolution quantitative computed tomography (HRQCT)-based  were more superior to DXA in discriminating between patients of differing prevalent vertebral fracture status(25).

V. The Diet
1. Green tea
Black tea polyphenol, theaflavin-3,3′-digallate (TFDG) and green tea, epigallocatechin-3-gallate (EGCG)  are found to be a lead compounds for the treatment of bone resorption diseases, through
inhibition of the formation and differentiation of osteoclasts, according to study.

2. Organic soy
  Ingestion soy isoflavone extracts (not soy protein or foods containing isoflavones)
supplements increased lumbar spine BMD in menopausal women, but not femoral neck, hip total, and trochanter BMD, according to study.

3. Milk thistle seeds

Silibin found abundantly in Milk thistle seeds has shown to  promote bone-forming osteoblastogenesis and encumber osteoclastic bone resorption in vitro cell systems of murine osteoblastic MC3T3-E1 cells and RAW 264.7 murine macrophages.

4. Skin and seed of grape
Resveratrol, a phytohemical of
skin and seed of grape promoted spontaneous osteogenesis(bone formation) but prevented adipogenesis(adipocytes in storage of fat) in human embryonic stem cell-derived mesenchymal progenitors, according to study. 

VI. Antioxidants and Osteoporosis

Dietary and endogenous antioxidants were consistently lower in patients with osteoporosis, according to University of Perugia, leading to decrease antioxidant defenses and promoting pathogenesis of osteoporosis(32).

2. Selenium plus vitamins E and C

In rabbits study, combination of vitamins E and C given partially prevented this bone tissue destruction on the progress of osteoporosis. When combine with sodium selenite given together with vitamins E and C to the osteoporosis model rabbits, the long bone tissue of osteoporosis rabbits had almost the same structure as in normal rabbits(33).

3. Vitamin C
According to the study epidemiologic studies, correlate low vitamin C intake and
genetic deletion of enzymes involved in de novo vitamin C synthesis are associated to bone loss and severe osteoporosis, respectively(34).

4. Calcium and vitamin D

Supplementations with daily doses of 400-800 IU of vitamin D, given alone or in combination with calcium, are able to reverse vitamin D insufficiency, to prevent bone loss and to improve bone density in the elderly, according to  University of Siena(35)

A. In conventional medicine perspective

A.1. Bisphosphonates
1. Including Alendronate (Fosamax), Risedronate (Actonel, Atelvia), Ibandronate (Boniva), Zoledronic acid (Reclast, Zometa), etc.. Bisphosphonates are antiresorptive medications widely prescribed for treatment of osteoporosis. The drugs have shown to significantly reduce the risk of osteoporotic fractures(36). Higher risk patients should be treated by
nonbisphosphonatefor 10 yr,  have a holiday of no more than a year or two(37).

2. Side effects
a. Nausea
b. Abdominal pain
c. Difficulty swallowing
d. Rrisk of an inflamed esophagus or esophageal ulcers(38)
e. Risk of scleritis and a variety of ocular side effects(39)

2. Hormone-related therapy 

Hormone replacement therapy can help to maintain bone density for menopause women, but it increases
a, The risk of breast cancer and heart disease(40)
b. The risk for venous thromboembolism(41)
c. The risk of (Nonmelanoma Skin Cancers) NMSC.(42)
d. The risk of stroke(43)

B. In herbal medicine perspective
1. Red clover
 

 Red clover extract (RCE) standardized to contain 40% isoflavones by weight (genistein, daidzein, biochanin A, and formononetin present as hydrolyzed aglycones) together with a modified alkaline supplementation have shown to improve OVX-induced osteoporosis while a mild metabolic alkalosis might further synergize some therapeutic aspects(44).

2.  Organic soy
Soybean protein showed to promote higher rate of bone turnover in fed animals, together with enhancement of ertebral and femoral bone densities, according to
nonbisphosphonate(45).

3. Soybeans, clover and alfalfa sprouts, and oilseeds (such as flaxseed)
In humans, animals, dietary phytoestrogens found in soybeans, clover and alfalfa sprouts, and oilseeds (such as flaxseed) play an important role in prevention of menopausal symptoms, osteoporosis, cancer, and heart disease(46).

C. In traditional Chinese medicine perspective

 Osteoporosis in elder is a condition of the drop of Kidney Jing. As we age, our kidney doesn’t have the energy to nourish the bones. The weak and brittle bone can, lead to the symptoms of earaches, ringing in the ears, hearing loss, hair loss, teeth problems, knee pain and lower back pain, loss of sex drive, including osteoporosis.

1. Du-Huo-Ji-Sheng-Tang and Du Zhong (Cortex Eucommiae) 
Du-Huo-Ji-Sheng-Tang and Du Zhong (Cortex Eucommiae) was the most frequently prescribed herbal formula and single herb, respectively, for the treatment of osteoporosis, according to the study by the
Department for Traditional Chinese Medicine, Chang Gung Memorial Hospital(47).


2.  Zuogui Pill
  Zuogui Pill showed effectively in preventions and treatments of glucocorticoid-induced osteoporosis in rats by up-regulating the expression of the key signal molecules inWnt signal transduction pathway(
pass signals from outside of a cell through cell surface receptors to the inside of the cell)(48).

3. Embedding thread at Shenshu (BL 23)
  Embedding thread at Shenshu (BL 23) was found to increase BMDs of hip and lumbar vertebrae in patinets with osteoporosis. The rate of bone fracture during 5 years after treatment was 2.1%  significantly lower than 18.2% in the medication group(49).


4. Shaoyang Meridians 
 Foot-Shaoyang Meradians can modulate bony strength under physiological and pathological conditions, and treat osteoporosis which mainly manifests as ostealgia and easy fracture, according to

                           Dementia

About 5-8% of all people over the age of 65 have some form of dementia, and this number doubles every five years above that age. Dementia is the loss of mental ability, severe enough to interfere with people's every life and Alzheimer's disease is the most common type of dementia in aging people.

C. In traditional Chinese Medicine Perspective(*)C.1.3. Radix glycyrrhizae(Gan Cao)
Gan Cao, a sweet and neutral herb, also known as Licorice root, has been used in TCM as anti allergy(752), anti inflammation(751), anti ulcer(753), anti convulsion(754) and to treat stomach weakness(755), tired and lack of strength(756), enhance cardiac performance(757) and short of breath(758), cough with abundance of phlegm(751), etc., as it tonifies the Spleen, benefits the Qi, moistens the Lungs, calms cough, deaf or acute pain, by enhancing the function of all 12 channels.

Phytochemicals
1. Glycyrrhizic acid
2. 24-hydroxyglycyrrhetic acid
3. Licorice saponins A3
4. Licochalcone A
5. Licochalcone B
6. Licoflavone
7. Liquiritin
8. Liquiritigenin
9. Isoliquiritigenin
10. Ononins
11. 4′,7-dihydroxyflavone
12. Etc.

  Gan Cao used in the treatment and prevention of dementia(719) in traditional Chinese medicine, may be due to their effectiveness of phytochemicals in ameliorated amyloid pathology and related cognitive deficits(759). In senile dementia, Gan Cao exhibited its elicit memory-improving effects, through its antioxidant, anti-inflammatory, antiapoptotic, neurogenetic, and anti-Aβ activities(760).

1. In Alzheimer's disease (AD)
Vascular risk factors (VRF) has shown strong evidence in contribution to cognitive decline and the development of Alzheimer's disease (AD)(763) and dementia(761), Sini Decoction, a TCM formula containing Gan Cao enhanced cardiac function through significant suppression of  lipid peroxidation(765) of myocardial homogenate(764), via scavenge oxygen free radicals activity(764). According to Korea Food Research Institute, Gan Cao attenuated the Amyloid beta protein (Abeta)  involved in the progression of Alzheimer's disease (AD), through inhibition of Acetylcholinesterase activity(762), probably via mediation antioxidant actions against oxidative stress(762).


2. In Parkinson's diseases(PD)
Kampo kami-shoyo-san (TJ-24), a Korean traditional herbal medicine, containing Gan Cao,  significantly reduce  tremor, disturbances in muscular movement in antipsychotic-induced parkinsonism(766). The formula also showed effectively in relieved symptoms of panic attacks, anticipatory anxiety(767). Liquiritin, a flavonoids extracted from Glycyrrhizae radix, significantly promotes the neurite outgrowth in dose dependant manners, through overexpression of neural related genes, such as neurogenin 3, neurofibromatosis 1, associated to Alzheimer's disease or Parkinson's disease(768).

3. In learning and memory
Oral administration of Glycyrrhizae radix (GR) in rat study, showed to reduce anxiety and promote learning and memory in rats, by attenuating the behavioral and neurochemical impairments caused by repeated stress-induced alterations(770). Palmul-chongmyeong-tang, containing Gan Cao used in used as herbal medicine against ischemia, may be a potential agent for treatment of vascular dementia as it significantly improved learning and memory and attenuated cognitive impairment through reduction of the loss of cholinergic immunoreactivity in the hippocampus induced by cerebral ischemia in rat model(769).

Serves 4
Simple and delicious
2 large leeks- just the white part- check for residue, then cut into cork tube-shape
salt and pepper
glass of white wine
3 tbsp water
50 g /2 oz. butter

1. Place the leek pieces in the shallow pan. Season with salt and pepper.
2. Pour into wine and pepper, then add the butter. Put on the lid and bring up to a simmer. Cook over the low heat for about 20 minutes p the leeks should be beautiful tender.

 Skin aging is one of the most visible aging processes occurred constantly in our skin. According to the Clinical Centre of Nis, certain plant extracts may have the ability to scavenge free radicals to protect the skin matrix through the inhibition of enzymatic degradation, or to promote collagen synthesis in the skin, and affect skin elasticity and tightness(1).
Dehydroascorbic Acid (DHA) is an oxidized form of vitamin C with Chemical Reaction and Structure as follows


According to Dr. Douglas Q. Kitt, Topical Dehydroascorbic Acid (DHAA) permeates stratum corneum at a rate up to 12 times faster than vitamin C. This supports the concept that lower concentrations of DHAA in topical preparations can enhance skin vitamin C levels with less potential side effects (2).
  
Liver spots or aging spots are condition of brown or black spots appearing on the surface of the skin due to aging and exposure to ultraviolet radiation from the sun. According to skin care guides, high concentration of Vitamin C may improve the skin and lighten the spots(3). Other articles insisted that liver spots may be the result of a deficiency of vitamin C(4). According to the Duke University Medical Center, the application of topical solutions containing vitamins C and E provided protection for human skin against damage caused by ultraviolet radiation(5)6)(7)(8).

Our previous study(8) indicated that Dehydroascorbic acid (DHA) 4% may effectively attenuate the roughness, scaliness and get rid off most aging spot and photo aging spots on facial skin in our 24 weeks study through it antioxidants, anti inflammation and free radicals scavenging activities.
Re examination facial skin of patients who continuously apply the formula, after 3 months showed a more profound effectiveness.

How to make Dehydroascorbic acid (DHA) 4% concentration
Day one
Put 8 tablets of synthetic vitamin C into the bottle which carries 100 mL of water[(8x500mg)/100mL=4000mg/100,000mg=4/100=4% concentration of Dehydroascorbic acid (DHA)] on the day before application to make sure all vitamin C tablets are dissolved.

How to remove Liver and Aging spots and re smooth the facial skin 
Liver spot or aging spot is a condition of brown or black spots appeared on the surface of the skin due to aging as the skin is less able to regenerate from sun exposure.
The next day
Put on your glove. Shake the bottle well. Pour some Dehydroascorbic acid (DHA ) onto a piece of fold-twice bounty paper towels. Apply the solution 3 times a days by rubbing against the skin of the effects area with little pressure  for about one minutes. Apply more solution if require.  Dehydroascorbic acid (DHA) 4% concentration should be able to lighten most of Liver and Aging spots and re smooth some of the skin on the effect areas in a few days.

After applying, clean the sponge if used and glove, return  and store the bottle of solution into first aid box in room temperature. Replace the solution every ten days to retain accurate concentration.

What to expect Dehydroascorbic acid (DHA) exhibited its detoxified, antioxidant, lightening and enhanced cell regeneration effects in getting rid of skin damage causes of aging liver and spots and lightening hyperpigmentation due to sun exposure.

Precautions 1.  Always testing your topical skin solution, including Dehydroascorbic acid (DHA )  for allergic reaction, by applying a tiny portion onto your wrist for 5 minutes. Do not use the solution if there is allergic reaction.
2. Although there were no adverse effect in our study, please make sure you always wear gloves before applying, as higher concentration of the solution has shown to brownness on the finger nails.
3. The solution has shown to cause yellowness on the surface of some metal (very old) sinks, please make sure your test it to avoid the same messes.
4. Higher concentration of the solution has been used in skin tanning industry,  please make the solution is completely absorbed(about 5- 10 minutes) into your skin before going to the extreme sun.
5. If the solution get into your eyes, use tap water to clear them.
6. Do not attempt to make higher solution and put them on your facial skin, as it has not been tested.

About 5-8% of all people over the age of 65 have some form of dementia, and this number doubles every five years above that age. Dementia is the loss of mental ability, severe enough to interfere with people's every life and Alzheimer's disease is the most common type of dementia in aging people.

In traditional Chinese Medicine Perspective(*)According to the Unilever R&D Shanghai, and Unilever R&D Vlaardingen, Dr. and the team of researchers, traditional Chinese medicine (TCM), in 3000 years' literary history, the top 5 TCM herb ingredients including Poria cocos(720), Radix polygalae, Radix glycyrrhizae, Radix angelica sinensis, and Radix rehmanniae have been used extensively for treatment of dementia.

C.1.2. Radix polygalae(Yuan Zhi)
Yuan Zhi, sweet and slightly cool herb is also known as Thinleaf Milkwort Root, used in TCM as antibiotic property, anti-viral property(732), anti cancer(734) and anti depression(733) agent in treating hepatitis B, tumors in the digestive system, respiratory system and in the uterus, etc., as it calms the heart, transforms phlegm,  resolves furuncle and oedema, expel toxin, etc. by enhancing the functions of spleen, lung and liver channels.

2. Polysaccharopeptides (PSP)

3. Tenuifolin
4. Onjisaponin B

Yuan Zhi used in the treatment and prevention of dementia(719) in traditional Chinese medicine, may be due to their effectiveness of phytochemicals in ameliorated amyloid pathology and related cognitive deficits(735). In aging related dementia, based on the history of use, and pharmacological investigation, Yuan Zhi showed a strong evidence in memory improving, including cholinergic, antioxidant, anti-inflammatory, antiapoptotic, neurogenetic, and anti-Aβ activities(736). Palmul-chongmyeong-tang, a traditional Chinese medicine formula containing Yuan zhi, exhibited improvement of learning and memory deficits, through reduction of loss of cholinergic immunoreactivity induced by cerebral ischemia(737).

 
 
1. In Alzheimer's disease (AD)
In free radical involved neuro-degeneration in Alzheimer's disease (AD), pretreament with Paeng-Jo-Yeon-Nyeon-Baek-Ja-In-Hwan (PJBH) prescription significantly exhibited cytoprotective effects, through elevating cell survival, antioxidant enzyme activities and resulted decreased production  of malondialdehyde (MDA)  which associated to Alzheimer's disease (AD)(738). Smart Soup (SS), composed of Rhizoma Acori Tatarinowii (AT), Poria cum Radix Pini (PRP) and Radix Polygalae (RP), in  oral administration of SS ameliorated the cognitive impairment of AD in animal model, effectively reduced Aβ levels, retarded Aβ amyloidosis and reduced Aβ-induced gliosis and neuronal loss(739).

2. In Parkinson's disease
Mutant Huntingtin gene caused by a poly-glutamine expansion in huntingtin( 740)and A53T α-synuclein, a protein found abundantly in the human brain, at the tips of nerve cells (neurons)(741) have shown to be associated with the development of Huntington disease and Parkinson disease. Yuan Zhi phytochemical, onjisaponin B  and ethanol extracts showed to induce autophagy(743) through signalling pathway in participation of regulation of vascular function(742) in protection  of cells against pro-neuro apoptosis  via the mitochondrial pathway(744). According to colege of Pharmacy and Kyung Hee East-West Pharmaceutical Research Institute, in mouse models of Parkinson's disease, Yuan Zhi  also exhibited it protective effects on dopaminergic neurons induced by toxicity(745).

3. In Huntington diseases
 Mutant Hungtintin caused of  degradation of long-lived cytoplasmic proteins, protein complexes (the gel-like substance enclosed within the cell membrane)(747), or damaged organelles(tiny structures that perform very specific functions within cells)(748) attentuated autophagy in asociateion of  the development of Huntington diseases(746), onjisaponin B, found in Yuan Zhi,  effectively induce  autophagy through its acceleration of both the removal of mutant huntingtin and and α-synuclein in PC-12 cells(749), which contain mixture of neuroblastic cells and eosinophilic cells(750).

Serves 4
The fresher the bean, the riper the tomatoes, the better the course. Try this as a starter.

1 glove of garlic- thinly sliced
2 tbsp olive oil
250 g/ 1/2 lb. cherry tomatoes - cut in half
salt and pepper
500 g/ 1 lb. green bean
1. First make the sauce, Saute the garlic lightly in the oil
2. Add tomatoes and cook for 10 minutes until the sauce look unctuous  but the tomatoes retain the bit of their shapes. Add salt and pepper to taste.
3. Bring a pan of water to boil and salt well. Add bean and cook until tender - bite them ti test them. Drain thoroughly and add to the sauce, turning them in it carefully.

C.1. 1. Poria cocos(Fu Ling)
Fu Ling, fu shen or fu ling pi also known as Poria, the bland, sweet and neutral herb has been used in TCM as diuretics(711) and to treat urinary difficulty(711), edema(711), diarrhea(715), emesis(717), vomiting(716), dizziness(716), heart diseases(717)(718), etc., as it eliminates water, strengthens the Spleen, calms the Mind, etc., by enhancing the functions of heart, spleen, lung and kidney channels. The herb has shown to exhibit various biological activities including anti-tumor(713), anti-inflammation(712), immunodepression(712) and antioxidant(711)(713)(714).

Phytochemicals
1. Pachymose
2. Albuninoid
3. Pachymaran
4. B-pachyman
5. B-pachymanase,
6. Pachymic acid,
7. Tumulosic acid,
8. Eburioic acid,
9. Pinicolic acid,
10. Etc.

Fu Ling(720) used for treatment and prevention of dementia(719) in traditional Chinese medicine, may be due to their effectiveness of phytochemicals in attenuation of oxidative stress in initiation of neurodegeneration and enhanced neuroprotection(719).

1. Learning and memory impairments
Kyung-Ok-Ko (KOK), the traditional Chine medicine formula, containing Poria cocos, showed effectively in exhibition of its anti-inflammatory effects in protection of brain neuron through attenuating memory(721)(722) and learning impairment(723). In senile dementia, herbal Poria cocos also elicited memory-improving effects in vivo and in vitro through its sinapic acid, tenuifolin, isoliquiritigenin, liquiritigenin, glabridin, ferulic acid antioxidants via cholinergic, antioxidant, anti-inflammatory, antiapoptotic, neurogenetic, and anti-Aβ activities(722).Yokukansan (TJ-54), another Chinese medicine formula, containing Poria cocos, also inhibited neurological disorders, including dementia and Alzheimer's disease(723) symptoms of memory and behaviors disturbance(724), probably through its anti-cholinesterase activity(725).

2.  Cognitive dysfunction
In patients of dementia with  cognitive dysfunction, Liuwei Dihuang decoction, containing Poria cocos, showed significantly in attenuated the neural apoptosis, inflammation and Aβ deposition in the hippocampus and cerebral corte(726) causes of cognitive impairment(727) in streptozotocin-induced diabetic rat. In spatial cognition, herbal formula Toki-Shakuyaku-San and fractions of Angelica acutiloba, improved memory and working memory impaired by scopolamine in animal study(728).

3. Alzheimer disease
In mouse model of Alzheimer disease, herbal formula Jangwonhwan, not only showed effectively in ameliorated AD-like pathology, but also exhibited its neuroprotective effects against Abeta-induced cell death, and the Abeta accumulation and plaque deposition in the brain(730), probably through attenuation of reactive oxidative stress(729), by suppressed Aβ-induced apoptosis and ROS production(731).
