Some Serious Complications of Dementia, Even Caregivers May Not Aware Of

About 5-8% of all people over the age of 65 have some forms of dementia, and these numbers double every five years above that age.

Dementia is the loss of mental ability that is severe enough to interfere with people's everyday life.

 And Alzheimer's disease is the most common type of dementia in aging people.

1. Pneumonia
In the study of Beth Israel Deaconess Medical Center, Boston, Massachusetts, Dr. Givens JL showed that Pneumonia is common among patients with advanced dementia, especially toward the end of life.

In those initially presenting with PE, the risk of dying of PE far outweighed that of fatal bleeding.

And,  in patients presenting with DVT alone, the risk of fatal PE was lower than that of fatal bleeding.(15).

Coffee Gobbles Up Arterial Plaque, in Reduced Mortality Risk of Myocardial Infarction (Heart Attack), Scientists Say

Habitual coffee consumed by myocardial infarction patients showed a significant reduced risk and rate of mortality, some scientists suggested.

Myocardial infarction is another term of heart attack occurred when blood flow suddenly decreases or stops to a part of the heart, that cause substantial damage to the heart muscle.

The causes of heart attack are associated to the building up plaque in the arteries, affecting the volumes of blood flood in transport nutrients and fluids to the body organs and tissue due to narrowing of the blood vessels.

Cholesterol narrowing the arteries is one common cause of coronary artery disease, a major cause of heart attack.

According to the National Heart, Lung, and Blood Institute (NHLBI), plaque is made up of fat, cholesterol, calcium, and other substances found in the blood.

People in the early stage of heart attack may experience symptoms of chest pain or discomfort which may affect upper body, including pain of the shoulder, arm, back, neck, or jaw.

Researchers at the Mayo clinic suggested, "Limiting how much saturated and trans fats you eat is an important step to reduce your blood cholesterol and lower your risk of coronary artery disease" and "A high blood cholesterol level can lead to a buildup of plaques in your arteries, called atherosclerosis, which can increase your risk of heart attack and stroke".

In the differentiation of the effect of fruit and vegetable intake and risk for coronary heart disease, Dr. Joshipura KJ at Harvard School of Public Health indicated
* Highest quintile of vegatables and fruits intake is associated to reduced risk of coronary heart disease with the relative odd ratio of .80 in compared to lowest quintile of intake.

* The risk of coronary heart disease is reduced by 4% for each 1-serving/d increase of fruits or vegetables. The odd ratio is decrease further to those who eat green leafy vegetables, regularly.

Coffee, a popular and social beverage all over the world, particularly in the West, is a drink made from roasted bean from the Coffea plant, native to tropical Africa and Madagascar.

In a study of 4365 Dutch patients from the Alpha Omega Cohort, aged 60-80 y (21% female) and experienced an MI <10 y before study enrollment, using baseline (2002-2006), to evaluate the dietary data including coffee consumption over the past month collected and 203-item validated food-frequency questionnaire, after examine the returned questionnaire from participants, researchers showed that
* Most of patients(96%) were coffee drinkers
*  Intake of coffee >4 cups/d, regularly and daily expressed a inverse association to risk of mortality in compared to 0-2 cups/d.
* Motility risk after myocardial infarction (MI) was decreased for every cup of additional coffee intake

Interesting, researchers also found out that there were no risk difference between caffeine and decaffeinated coffee.


These findings suggested that the efficacy of reduced risk of mortality of MI patients may be associated to chemical constituents in coffee, other than caffeine.

Additionally, in the review of literature in the data base of MEDLINE and Embase published between 1946 and 2015, 2 eligible studies involved-AMI mortality risk against coffee consumption with a total of 3271 patients and 604 died, in patients classified as light coffee drinkers (1-2 cups/day) versus noncoffee drinkers, heavy coffee drinkers (a>2 cups/day) versus noncoffee drinkers and heavy coffee drinkers versus light coffee drinkers, scientists filed the following results
* All groups showed a significant reduction in risk ratio in compared to non drinkers.

* The hazard risk ratio was 0.79 in light coffee drinkers versus noncoffee drinkers.

* The risk ratio of 0.54 was found in heavy coffee drinkers versus noncoffee drinkers and

* Heavy coffee drinkers versus light coffee drinkers were associated with a risk ratio of 0.69


Although there was no linear association of amount of coffee intake in risk of MI and MI morality, coffee intake expressed a strong effect against risk of MI and MI death in patients after heart attack.

These results of coffee efficacy may be associated to coffee hypercholesterolemic activity in prevention of cholesterol building up for the development of coronary artery disease(5).

Most important, eventhough habitual coffee consumption exerted a enormous effect in reduced risk
coronary heart disease, congestive heart failure, arrhythmias, and stroke as well as decreasing risk of mortality, but most of the data on coffee's health effects are based on observational examine with very few randomized, controlled studies, and association does not prove causation.

There fore, further data collection on studies performed with human consumption over a long term in compared coffee and non coffee intake ( risk of heart attack)and during the course of the disease(risk of mortality) will be necessary to complete the picture of  possibilities of coffee efficacy.

Eggplant may be used as a therapeutic and positive anti inflammatory functional food without alternating the immune activity in inflammatory diseases and conditions, some scientists opinionated.

Eggplant with deeply purple colcor, is a species of S. melongena, belongs to the family Solanaceae cultivated in southern and eastern Asia since prehistory, and native to India.

Inflammation is a natural expression by immune system in induction of while blood cells for protection our body against infection caused by foreign invasion.

However, Over stimulation of inflammatory cytokines to fight the infection may have strong negatively impact on the body tissue and organs, such as forming of scars, death of the healthy cells in the infectious site.

Prolong period of inflammation may lead permanently tissue damage and alternation of DNA of healthy cells.

Some researchers suggested that chronic inflammation can eventually lead to development of numbers of diseases and conditions, including some cancers, diabetes and rheumatoid arthritis.

(Solanaceae) effect in induction of antioxidant, analgesic, hypolipidemic and antiallergic activity in mice induced by injection of trypsin or trans-cinnamoyl-LIGRLO-NH(2) (tc-NH(2)) into the hindpaw, scientists found that oral administration of the extract at doses of 5, 10, and 100 mg/kg display strong effects in inhibition of both paw edema and vascular permeability.

Further analysis also showed that the highest dose injection of 10 mg/kg expressed a greatest activity in compared to the others in significantly inhibited PAR2 agonist-induced myeloperoxidase (MPO) activity in stimulated immune function activated pro inflammatory cytokines and tumor necrosis factor (TNF)-alpha expression to precipitate inflammation in response to the acute phase of infection.


Proteinase-activated receptor-2 (PAR2), are a family of four G-protein-coupled receptors found in the inflammatory cells and epithelial tissues that process a function in participated in the development of inflammatory diseases.


In these cases, Proteinase-activated receptor-2 (PAR2) induced inflammation through activated pro inflammatory cytokines, such as tumor necrosis factor (TNF)-alpha involved systemic inflammation in the acute phase reaction.

Additional differentiation, also suggested that the doses of 200mg/kg and 400 mg/kg of aqueous extract of S. melongena L showed the percentage of inhibition of 42.62% which is less than the standard drug aspirin which showed 64.5% inhibition.


In free radical induced chronic inflammation, the investigation of 4 our new lignanamides, melongenamides A-D (1-4), together with six known ones (5-10) isolated from the roots of Solanum melongena L. also demonstrated a significant activity in inhibitions of inflammatory effect caused by free radical species nitric oxide production in lipopolysaccharide-induced RAW 264.7 macrophages through their antioxidant effect.

Interestingly, the study also found out that not all compound extracted from the roots expressed the same levels of antioxidant properties in reduced inflammatory induced by ROS.

In compared to other compounds, compound melongenamide 1  exerted antioxidant effect without inhibited free radical nitric oxide production.


The findings suggested that eggplant may be used as function food in reduced risk of inflammation in acute infection and as a adjunct therapy in combination with standard treatment of chronic inflammatory diseases.

However, further data collection on studies performed with human consumption during the course of the diseases and conditions are necessary to complete the picture of eggplant anti inflammatory  possibilities.

Appendicitis:The Causes and Risk Factors

Appendicitis is a condition characterized by inflammation of appendix.

It is classified as an emergency, in many required the removal of the appendix.

If bursted, or perforated, spilling infectious materials into the abdominal cavity, can be life threatening.

A. Causes
The cause of appendicitis is the result of blockage by stool, a foreign body, or cancer of that can lead to infection.
1, Bacterial, viral and parasitic infiection
According to the study by the, the pathologic spectrum of the inflamed appendix encompasses a wide range of infectious entities, some with specific histologic findings, and others with nonspecific findings that may require an extensive diagnostic evaluation.

 The appendix is exclusively involved in some of these disorders, and in others may be involved through extension from other areas of the gastrointestinal tract(4).

Other in the study to investigate the role of parasitic infestation in the etiology of acute pediatric appendicitis, indicated that parasites are present in 5.5% (88 patients), and of those 88 parasitic infestations, 45 (51.1%) are Enterobaisis, 8 (9.1%) are Schistosomiasis, 23 (26.1%) are Ascariasis, 7 (8%) Trichuriasis, and 5 (5.7%) are Teania Saginata.

The percentage of patients with suppurative, gangrenous or perforated appendicitis was similar in both groups with no statistical significance, irrespective of the presence or absence of parasitic infestation(5).

2. Foreign objects
Appendicitis and its complications remain a common problem affecting patients of all age groups.

 Appendicitis due to foreign bodies is rare and carries an estimated frequency of 0.0005%.

But according to the Department of Surgery, Heilig Hart Tienen, there is a case of a 44-year-old man with appendicitis induced by an appendicolith containing eight steel shotgun pellets.

The man was a recreational hunter and for the past 20 years he consumed wild game on a regular basis(6).

B. Risk factors
1. Age 10 × 103/mm
In the study to evaluate the impact of timing of appendectomy and other potential risk factors on progression of acute appendicitis, by searching the relevant databases of a tertiary medical center identified 1,604 patients with verified acute appendicitis who underwent appendectomy in 2004-2007with demographic and clinical data and time from symptom onset to emergency room admission (“patient interval”) and from emergency room admission to surgery (“hospital interval”) and their combination were analyzed by pathological grade, researcher found  that on multivariate analyses, independent risk factors for appendiceal perforation were age 50 years, white cell count >10 × 103/mm(3), body temperature >37.8 °C, hospital interval >24 h, patient interval >48 h , and combined interval >48 h.

2. Gender difference, among young
According to study appendicitis is common among young, healthy populations; appendectomy is one of the most common surgical procedures performed in the United States.

Among active and reserve component members, there were 31,610 cases of appendicitis and 30,183 appendectomies during 2002 to 2011.

The overall incidence rate of appendicitis in the active component was 18.4 per 10,000 person-years (p-yrs).

Active component males reported greater rates of perforated appendicitis (2.6 per 10,000 p-yrs).

Active component females had higher rates of incidental appendectomies (2.6 per 10,000 p-yrs)(8).

3. Race, increased over time and is higher in the summer months
Appendicitis is most common in whites and Hispanics and less common in African Americans and Asians.

And incidence has increased over time and is higher in the summer months, according to the study by the University of California San Diego(9).

4. Prior antibiotic administration
Prior treatment with antibiotics was an independent risk factor for therapeutic delay in pediatric AA, according to the study by the National Center for Child Health and Development, Tokyo(10).

5. Factors associated with an increased likelihood of appendicitis included decreased bowel sounds; rebound tenderness; and presence of psoas, obturator, or Rovsing’s signs(11).

6. In patients with end-stage renal disease
The independent risk factors were atrial fibrillation (hazard ratio [HR], 2.08), severe liver disease (HR, 1.74), diabetes mellitus (HR, 1.58), and hemodialysis (HR, 1.74), according to the study by the Taipei Medical University(12).

7. Severity of inflammation
CRP concentration may be a potent objective predictor of pathological severity in appendicitis. Combination with the other diagnostic modalities may improve the diagnostic accuracy in predicting the severity of appendicitis(13).

8. Appendicolith
Presence of an appendicolith was associated with a 72% rate of recurrent appendicitis compared with a recurrence rate of 26% in those with no appendicolith.

9. Other risk factors
The principal factors contributing to perforation of appendix are: age of children, delays of surgical intervention, family anamnesis, social group and late recognition of symptoms of appendicitis(14).

Candida albicans are members of a large group of micro organism whose cells contain complex structures enclosed within the membranes, including yeast(2)(3), fungi(4)(5)(6), and mold(6) that live among the gut flora in the human mouth and gastrointestinal tract.

Non-albicans Candida (NAC) species cause 35-65% of all candidaemias in the general patient population(1).

According to joint study, in many cases, biofilm(microorganisms with cells stick to each other on a surface) formation(7) gene mutations(8) and overexpression of genes(9)(10) are often associated with increased candida resistance toward antifungal drugs.

ROS species have duo functions that are well documented for playing an important role in our body as both beneficial and deleterious molecules in expression of cytotoxicity against abnormal cells alternation and initiation of chronic ailments by causing imbalance of the ratio among antioxidants and free radicals, respectively.
