                         Musculoskeletal disorders (MSDs)


Musculoskeletal disorders (MSDs) is medical condition mostly caused by work related occupations and working environment, affecting patients’ muscles, joints, tendons, ligaments and nerves and developing over time.
Most common Musculo-Skeletal Disorders include
1. Musculoskeletal pain .
2. A shoulder disorder
3. rheumatoid arthritis and osteoarthritis (OA) of the hand, hip, and knee.

Types of Musculo-Skeletal disorders in ages of 50 plus(2)
1. Osteoarthritis

3. Rheumatoid Arthritis
4. Polymalagia Arthritis
5. Cervical myleopathy and spinal canal stenosis
6. Osteoporosis
7. Low back pain


                                        Osteoarthritis

Osteoporosis is condition of thinning of bone and bone tissues as a result of  the loss of bone density over a long period of time. 

I. Symptoms
1. Lower back pain, as a result of fractured or collapsed vertebra

People with Osteoporosis are susceptible to lower back pain than those without osteoporosis, according to a cross-sectional study, conducted from October 2002 to March 2003 to gather data from 24,435 adults aged 20 years and older selected randomly from Taiwan’s general population. The Osteoporosis rate of frequent low back pain in association with osteoporosis in men and women were 5.77 and 3.49 respectively(1).

2. Loss of height over time

Vertebral fracture cause of spinal core damage is associated to significant height loss in patients with osteoporosis, according to a study of 231 men and women over the age of 65 underwent DXA scan of their spine and hip (2).
  Osteoporosis patients with chronic obstructive pulmonary disease (COPD) are found to associate to excessive height loss(3).


3. A stooped posture
Postural deformity of patients with osteoporosis is associated to contributed risk factor for postural instability and falls.  Spinal curvature on postural instability in patients with osteoporosis may
influence lumbar kyphosis(lower back curvature)  in spinal inclination(4).

4. Easy bone fracture 

Easy bone fracture  may be results of a gradual loss of bone density.
 Intravenous infusion of zoledronic acid (5 mg) at 12 months, found that Zoledronic acid treatment was associated with a significantly reduced risk of vertebral fracture(
fractures in the spine, hip and wrist), among men with osteoporosis, according to a multicenter, double-blind, placebo-controlled trial of randomly assigned 1199 men with primary or hypogonadism-associated osteoporosis who were 50 to 85 years of age(5).

5. Neck and low back pain
  Neck and low back pain,
frequent in female are prevalent and highly associated in patients with headache and osteoporosis, according to 1-year prevalence of neck pain and low back pain in the Spanish population(6).

6.  Depression

Spine BMD was negative correlations between anxiety, stress, but depression was found to have a unique significant contribution to the explained variance in right and left hip BMD(7).

7. Other symptoms
Women with osteoporosis  re more likely to experience certain
psychosomatic, gastrointestinal and swelling and vasomotor symptoms, especially depressive symptom if compared to women in generally good health(8).

Cream green, somewhat tart and so easy.

Lime flavored gelatin 1 x 3 oz. 1 x 85 g
Boiling water 1 cup 225 ml
Crushed pineapple and juice 14 oz. 398 ml

Sour cream 1 cup 250 ml

Put gelatin in bowel. Stir in boiling water dissolve. Add pineapple with juice. Stir. Chill until syrupy.

Stir sour cream into thickened gelatin. Pour into mold or serving bowl. Chill. Serves 8.

1. According to Thessaloniki Eye Studies by the Aristotle University of Thessaloniki(15), and a Russian study(14) in assessment of the prevalence of primary angle-closure glaucoma (PACG, the risk factors include
a. Intraocular pressure (IOP)
b. Presence of pseudoexfoliation syndrome(PEX)
c. History of coronary artery bypass or vascular surgery
d. Moderate-to-high myopia.
e. Age
f. History of diabetes treated with insulin
 
2. Medical conditions and medication use
Certain medical conditions, such as diabetes, heart diseases, high blood pressure and hypothyroidism and medication  and used of medication such as such as corticosteroid for a prolonged period of time are associated with increased risk of Glaucoma(14a).

4. Other risk factors
Other risk factors such as gender, race, refractive error, heredity and systemic factors may play a role in glaucoma pathogenesis16).

A good, fluffy mint green color salad- a disguise for vegetable.

Lime flavored gelatin 1 x 3 oz. 1 x 85 g
Boiling water 1 cup 225 ml
Salt 1/4 tsp. 225 ml
Granulated sugar 1 tbsp. 1 ml
Cold water 1/2 cup 125 ml
Salad dressing (or mayonnaise) 1/2 cup 125 ml

Grated cabbage 2 cup 500 ml
Grated carrot 1/2 cup 125 ml
chopped celery 1/2 cup 125 ml
Copped peeled cucumber 1/4 cup 50 ml
Grated onion ( or 1 tsp. , 5 ml, onion flakes) 1 tbsp. 15 ml

Dissolve gelatin in boiling water in bowl. Stir in salt and sugar. Add cold water.
Whisk or beat in salad dressing to combine. Chill until slightly thickened. Beat with rotary beater until fluffy.

Fold in cabbage, carrot, celery, cucumber and onion. Pour into mold or serving bowl. Chill. Serves 8

Note: Gelatin may be completely set then  put in a blender to fluff before adding vegetables. It will make it nice and airy but it won't increase the volume as much when using a rotary beater.

1. Genes
a. Certain genes mutation, such as Myocilin(MYOC), Cytochrome P450, family 1, subfamily B, polypeptide 1(CYP1B1), and Forkhead box C1(FOXC1)
In primary Glaucoma, Matrix metalloproteinases (MMPs) play an important role on cell behaviors such as cell proliferation, migration (adhesion/dispersion), differentiation, angiogenesis, apoptosis, and host defense(23), defect of MMPs (MMP1 rs1799750 (-1607 1G/2G) and MMP9 rs17576 polymorphisms(25)) have been shown to associated with gender-dependent risk factors for developing primary open-angle glaucoma (POAG) and primary angle closure glaucoma (PACG), respectively(24)(26).
According to studies,
a.1. Although it is rare, genetic mutation of genes cytochrome P450, family 1, subfamily B, polypeptide 1(CYP1B1), myocilin (MYOC), optineurin(OPTN), and Opticin(OPTC) is associated to increased risk in Indian population(28).;
a.2. Mutation of  genes myocilin (MYOC) and optineurin (OPTN) sequence variations is associated to  risk factor in Spanish(29) and
a.3. Mutation of myocilin (MYOC),optineurin (OPTN), WD repeat domain 36 (WDR36), and apolipoprotein E (APOE) genes are increased risk of primary open-angle glaucoma (POAG) in northern Chinese(30).
a.4. Although other gene mutations, such as Latent Transforming Growth Factor Beta Binding Protein(LTBP2) has been found in north Indian patients with PCG, it is not involved in the pathogenesis of primary congenital glaucoma(27).


b. Connective tissue growth factor (CTGF)
The most critical risk factor for optic nerve damage in cases of primary open-angle glaucoma (POAG)(31) is an increased intraocular pressure (IOP) due to alternation of tissues in the eye located around the base of the cornea(32). Actin, a critical player in many cellular functions, including cell motility and the maintenance of cell shape, etc.(33), disruption or taken over actin cytoskeleton by numerous pathogens(34), may induce a resistance to aqueous humor outflow in the trabecular meshwork (TM)(36), an area of tissue is responsible for draining the aqueous humor from the eye via the anterior chamber causes of optic nerve damage involved connective tissue growth factor (CTGF), found in higher amounts in the aqueous humor of patients with POAG(35).

c. Alterations of serine protease
Angle-closure glaucoma (ACG) is a subset of glaucoma affecting 16 million people. According to the study by the The Jackson Laboratory, alterations of  serine protease, a family of enzymes that cut certain peptide bonds in other proteins with functions, especially in digestion, blood clotting, and the complement system(37) may induce a wide range of  ocular conditions, including reduced ocular size and ACG(38)(39) and high hyperopia(39).

2. Iactive congenital syphilitic interstitial keratitis
Iterstitial keratitis(IK), a condition of corneal scarring caused by chronic inflammation of corneal stroma(40) has been found to associate to the development of angle-closure glaucoma in the absence of corneal opacity in patinets(41).

3. Anatomical disorder(42)
Chronic angle-closure glaucoma, the commonest form of glaucoma in Sino Mongaloid populations can lead to permanent closure of part of the filtration angle in the anterior segment(42)(43) of the eye, leading to conditions of recurrent pupillary block and “creeping” angle closure(44).

4. Atypical ocular ischaemia
 Low intraocular pressure in untreated glaucoma(45) can lead to ciliary shut down, retinal detachment, post filtering surgery and ocular ischaemia, according to Glaucoma Services, LV Prasad Eye Institute(46).

5. Other causes
According to the examination of post operation patients with glaucoma in the first 275 consecutive eyes, other causes include
a. Exacerbation of a pre-existing glaucoma
b. IOP elevation induced mechanical angle closure, due to impaired ocular blood flow(48)
c. Steroid response(49)(50)
d. Peripheral anterior synechiae(51)(52)
e. Without detectable cause (47).

Lime flavored gelatin (jelly powder) 1 x 3 oz. 1 x 85 g
Boiling water 3/4 cup 175 ml
Salt 1/4 tsp. 1 ml
Cold water 3/4 cup 175 ml

Grated cabbage 1 cup 250 ml
Grated carrot 1/2 cup 125 ml
Diced celery 1/2 cup 125 ml

Dissolve gelatin in boiling water in bowl. Stir salt. Add col water. Chill until slightly thickened.

Add cabbage, carrot and celery to thickened gelatin. Fold in. Pour into mold. Chill Garnish with sliced cucumber or other vegetables. serves 8.

Glaucoma is a medical condition of an eye disease caused by induced retinal ganglion cell types
alterations before detectable dendritic loss. If untreated, the disease can lead to permanently vision impair and blindness.

I. Types of glaucoma(a)
There are several types of glaucoma. The two main types are (Chronically)open-angle(2)(3) and (Acutely)angle-closure(4)(5) both caused by either increase of intraocular pressure (IOP)(1), or pressure inside the eye(1).

A. Open-Angle Glaucoma affecting about 2.2 million Americans(6) and over 90% in all glaucoma cases(7), is the most common form of glaucoma, according to the 2000 statistic, with rates increased with ages(7).
Due to gradually progressive visual field loss, and optic nerve changes of the disease(9), patients may not experience any symptoms in the early stage of the diseases(10) depending to IOP high-tension and larger IOP fluctuations in normal-tension of OAG patients(8). Lacerating injury of cornea with iris prolapse in the eye if left untreated, may also induce open angle glaucoma(11).

A.1. Open-Angle Glaucoma caused by the slow progression of clogging of the drainage canals(13)  induced primary open-angle glaucoma (POAG) and pseudoexfoliative glaucoma (PEXG), due to increased intraocular pressure (IOP)(12).

A.2. Open-Angle Glaucoma is a result of the area  between the iris and cornea (The angle) widen(11).
The untreated rate of progression of patients with normal-tension glaucoma (NTG), and pseudoexfoliation glaucoma (PEXG) corresponded to advancing from normal visual function to blindness in approximately 70 years(14).

B. Angle-Closure Glaucoma
Angle-closure glaucoma is a less common form of acute glaucoma. Patients with angle-closure glaucoma may be experience the symptoms of sudden ocular pain, seeing halos around lights, red eye, very high intraocular pressure, nausea and vomiting, suddenly decreased vision, and a fixed, mid-dilated pupil(15), etc.
1. Caused by Sudden rise in intraocular pressure
Psychotropic agents(16)(17)(18) have been found to be frequently associated with ocular adverse effects(21) in inducing  angle-closure glaucoma, including pseudoexfoliation glaucoma caused by intraocular pressure (IOP) increased within hours after carbamazepine and gabapentin intake(19).

2. Caused by a closed or narrow angle between the iris and cornea(20)
Closed or narrow angle between the iris and cornea may be associated to early stage of acute angle-closure glaucoma(22).
