#Ginger #ERxtract Exhibits Anti #MetabolicDysfunction in Obesity in Vivos, Researchers Find

Kyle J. Norton

Scientists may have found a pungent spice that processes the potential to reverse metabolic dysfunction, according to studies.

Metabolic dysfunction is a medical condition characterized by the improper function of the liver or pancreas.

In some cases, metabolic dysfunction may also be caused by enzyme deficiency due to a defective gene, including Antitrypsin (AAT) deficiency, a common metabolic liver disease that affects infants, children, and adults.

The liver, the large organ in humans plays a critical role in breaking down old or damaged blood cells. and a central role in all metabolic processes, including fat metabolism by breaking down fat to energy for the body's needs.

On the other hand, the pancreas also converts the food we eat into energy for the body's cells by regulating blood glucose through the production of insulin.

Dysfunction of one of the 2 organs not only strongly impacts the body's energy levels but also increases the risk of diabetes and nonalcoholic liver disease.

According to the statistics provided by the Centers for Diseases Control and Prevention, in the US, among adults aged 18 years or older, the prevalence of metabolic syndrome rose by more than 35% from 1988–1994 to 2007–2012, increasing from 25.3% to 34.2%. The rise of metabolic dysfunction is probably associated with widespread obesity in the general population, some researchers suggested.

The risk factors associated with the onset of metabolic dysfunction include workers exposed to natural asbestos and mixed asbestos stock dust, improper nutrition, excessive intake of food with high content of free sugar, saturated fatty acids, and common salt lack of chicken meat, cereals, fish, vegetables, fruits, and cultured milk foods in the diet and excessive body weight.

Most people with metabolic dysfunction do not experience any symptoms. However, if you have a large waist circumference and experience symptoms of high blood glucose, increased thirst and urination, fatigue, and blurred vision, you may have a metabolic disorder. Please check with your doctor to rule out the possibility.

Ginger (Zingiber officinale) or ginger root, the second superfood used for thousands of years by mankind, is the genus Zingiber, belonging to the family Zingiberaceae, native to Tamil.

The root has been used in traditional and Chinese medicine for the treatment of dyspepsia, gastroparesis, constipation, edema, difficult urination, colic, etc.

Researchers on finding a natural compound for the modulation of metabolism evaluated the effects and the mechanisms of ginger extract intake in the metabolic disturbances animal model. Mice selected to the study were induced metabolic dysfunction by a high-refined carbohydrate (HC) diet.

Ginger extract at doses of 200, 600, and 1800 mg/kg supplemented in the daily food of obese Balb/c mice for an 8-week showed a metabolic protective effect by preventing the increase of adiposity induced by HC diet, improving lipid profile, and promoting the decrease of inflammatory markers.

Furthermore, treated mice also exerted the decrease in the recruitment of cells visualized in vivo in the microvasculature of adipose tissue, a decrease of inflammatory cytokines, and an increase of adiponectin serum levels.

The finding suggested that ginger process anti metabolic dysfunction in obese mice induced by a long-term high-refined carbohydrate (HC) diet.

In order to reveal more information about ginger anti-metabolic syndrome activity, used in traditional medicine, researchers examined the herbal effects on hyperlipidemia, hyperglycemia, oxidative stress, and inflammation.

Ginger showed a potency activity in reducing the onset of metabolic syndrome through
mediating the several transcription factors associated with the metabolic function, including activated glucose and fatty acid uptake.

The results indicated the beneficial effects of ginger on obesity and related complications in MetS.

Taken altogether, ginger may be considered a functional remedy for preventing and treating metabolic dysfunction, pending the confirmation of a larger sample size and multicenter human study.

3-n-Butylphthalide (NBP) Found in #ChineseCelery Enhances the Development of New Blood Vessels to the Affected Area for Dementia

Kyle J. Norton

Celery may have a potential effect on the protection of neuron integrity against the onset of dementia, scientists found.
Dementia is a group of diseases associated with a decline in memory or thinking skills severe enough to affect a person's ability to perform everyday activities and quality of life.

According to the statistic provided by the UK Alzheimer's Association, 1 in 3 people born in the UK this year will develop dementia in their lifetime.

Today, dementia affects over 850,000 people in the UK and this number will increase to over one million by 2025 and over two million by 2050. Alzheimer's disease accounts for 60 to 80 percent of all cases.
,
There are many types of dementia, depending on the specific regions damaged which are responsible for the differentiation of the brain's functions (for example, memory, judgment, and movement). Dementia patients may experience different symptoms as cells in that particular region can no longer perform their functions normally.

Alzheimer's disease is a brain disorder named after German physician Alois Alzheimer. Alzheimer's destroys brain cells, causing problems with memory, thinking, and behavior severe enough to affect language communication, memory, lifelong hobbies or social life. Alzheimer's gets worse over time, and it is fatal.

Absence of acetylcholine
The diminished quality of acetylcholine may result in cognitive dysfunction, causing language difficulty, memory loss, concentration problem, and reduced mobile skills because of lacking reaction in muscular activity and reflection.

Multi-infarct dementia
Also known as vascular dementia, is the second most common form of dementia after Alzheimer's disease in older adults, caused by different mechanisms involvingproblems all vascular lesions in the brain.

Parkinson's disease
Parkinson disease (PD) is a disabling, progressive condition, caused by the interruption of frontal-subcortical loops that facilitate cognition and parallel the motor loop due to the loss of substantia nigra pars compacta (SNc) dopamine (DA) neurons.

Creutzfeldt-Jakob disease
Creutzfeldt-Jakob disease is a quickly progressing and fatal disease that consists of dementia, muscle abnormal functions.

There are many risk factors associated with the prevalence of dementia. In the Western World, some researchers suggested that the widespread of dementia over the last many decades was probably attributed to the promotion of a poor diet.

Dr. P.J. Smith, the lead scientist in the examine the risk of cognitive decline associated with a poor diet said, " It has been recognized that lifestyle factors including dietary patterns, may be important in the prevention of cognitive decline and dementia in later life".

These results clearly suggested that people who follow a poor diet pattern are at substantial risk to develop dementia when they get older.

Celery is a species of Apium graveolens, belonging to the family Apiaceae. The plant is cultivated all around the globe as a vegetable. Celery can grow to 1/2 m tall with stalks (leaf on the top) arranged in a conical shape joined at a common base.

With an aim to discover a natural ingredient for the prevention and treatment of dementia with no side effects, researchers at The Chinese University of Hong Kong looked into the 3-n-Butylphthalide (NBP), a compound extracted from Chinese celery used as an anti-hypertensive herbal medicine for treating stroke patients.

In vitro and in vivo, 3-n-Butylphthalide (NBP) enhanced cell survival regardless of whether they are added before, simultaneously with, or after the addition of H(2)O(2).

In spontaneously hypertensive rats and Wistar Kyoto control rats with chronic cerebral ischemia, the application of NBP showed a significantly improved performance in the place navigation test and spatial probe test, compared to the untreated animals.

Additionally, in the chick's embryonic chorioallantoic membrane researchers found that NBP enhances the development of new blood vessels to the affected area accompanied by increased expression of growth factors, VEGF, VEGF-receptor, and bFGF, potent angiogenic inducers.

Dr. Zhang L, the lead author said, "One of the mechanisms of DL-NBP might be ameliorating vascular dementia and promoting angiogenesis".

In Alzheimer's disease (AD), the application of L-3-n-butylphthalide (NBP), an extract from seeds of Apium graveolens Linn (Chinese celery), showed a strong neuroprotective effect on ischemic, vascular dementia, and amyloid-beta (Abeta)-infused animal models.

In a triple-transgenic AD mouse model (3xTg-AD) that develops both plaques and tangles with aging, and cognitive deficits, 15 mg/kg L-NBP by oral gavage for 18 weeks significantly improved learning deficits, and long-term spatial memory, compared to control.

NBP treatment significantly reduced total cerebral Abeta plaque deposition and lowered Abeta levels in brain homogenates probably through enhancing the soluble amyloid precursor protein secretion (alphaAPPs), alpha-secretase.

More importantly, L-NBP treatment in 3xTg-AD mice also reduced glial activation in the induction of pathological pain and oxidative stress compared to the control.

The findings indicated NBP can be a promising preclinical potential of a multitarget drug for the prevention and/or treatment of Alzheimer's disease.

Taken altogether, Chinese celery processed a major compound 3-n-Butylphthalide (NBP) may be considered a functional food for the prevention and treatment of dementia, particularly in AD with no side effects.

#Honey #Eardrop Shows a Significant Effect on the Treatment of #EczematousExternalOtitis, Researchers Show

Kyle J. Norton

Acute otitis externa is a condition characterized by the inflammation of the ear canal. Most cases of acute acute otitis externa are associated with the infection by Pseudomonas aeruginosa and Staphylococcus aureus due to improper cleaning.
However, people with seborrhoeic dermatitis caused by inflammation of greasy areas of the skin can also lead to infection of the ears.

The most common symptoms of acute otitis include itching, canal edema, canal erythema, and otorrhea.

Most cases of acute otitis externa are healed within days with proper ear drops. However, there is a rare case that the disease may take weeks or months to recover.

Chronic otitis externa shares the common symptoms of the acute form, accompanied by water retention and severe ear pain, and temporary loss of hearing, due to building up thick and dry skin can build up inside the ear canal, leading to narrowing the ear canal.

Untreated chronic otitis externa can cause deafness.

Besides sharing the risk factors with the acute form, otitis externa can also be caused by infection due to the long-term use of hearing aids and earplugs.

Unlike the acute form, most cases of otitis externa are treated with steroid drops in addition to antibiotics accompanied by oral intake of pain medications such as ibuprofen.

Generally, oral antibiotics are not recommended, however, if the infection has spread beyond the ear canal, oral antibiotics may be applied.

According to the statistics, otitis externa affects 1–3% of people in America a year, and 95% of people's complaints of the condition are acute.

Honey, the rich golden liquid is a miraculous product made by bees using nectar from flowers.

The liquid is considered one of the healthy sweet food for replacing the use of white sugar and artificial sweetener by many people.

In finding a natural ingredient for the treatment of eczematous external otitis, scientists investigated the antibacterial potential of medical honey eardrops as a treatment of eczematous external otitis.

15 patients diagnosed with recurrent eczematous external otitis were selected and treated with medical honey eardrops for 2 weeks.

Honey treatment resulted in less discomfort and itching and decreased signs of eczema with no side effects, according to assays.

Although honey eardrops showed a strong in vitro inhibitory activity against all tested strains, they did not eradicate Staphylococcus aureus infection.

Honey eardrops also reduced the visual analog scale of ear complaints, a score of eczema, and increased the eradication of bacterial infection. Most patients in the study, are satisfied with the use of honey eardrops for the treatment of their eczematous external otitis.

Dr. Henatsch D, the lead scientist, after taking into account co and confounders said, "The results of this preliminary study indicate a possible role of honey eardrops in eczematous ear disease".

Taken altogether, honey may be considered a functional remedy for the treatment of eczematous external otitis, pending the confirmation of a larger sample size and multicenter human study.

#Herbal #Turmeric's Curcumin Protects Your Skin Against Photo-Damage through Its Antixodaint and DNA Repair Activity, Researchers Say

Kyle J. Norton

Photodamage is a type of skin damage caused by long-term exposure to sunlight or tanning beds.
Photodamage skin is one most common forms of skin damage in the US, due to misunderstanding the benefits and adverse effects of light exposure.

Believe it or not, a quarter of lifetime sun exposure occurs before the end of the teenage years. Most cases of photo skin damage can also stimulate some unwanted skin conditions, including advanced aging or wrinkling, uneven skin, hyperpigmentation, telangiectasias, actinic keratoses liver, and age spots.

In severe cases, long-term photodamage skin can also lead to skin cancer. More people in the US are diagnosed with skin cancer each year than all other cancers combined.

According to the statistic, 1 in 5 Americans will develop skin cancer by the age of 70.

Conventionally, most cases of early-stage and mild damage to the skin are treated by topical Levulan that removes scaly patches of actinic keratoses from your skin.

However, Laser surgery may be necessary, if the topical fails.

Dr. Jee SH, the lead scientist in the examination of the risk of photodamage skin in the risk of skin cancer wrote, "Some workers in paraquat manufacturing, exposed to bipyridines, have developed pigmentation and keratosis on sun-exposed skin".

And, "This pathologic study proves that all the lesions showed either photodamage or skin cancer. The strong trend in the correlation between the severity of photodamage and exposure to bipyridine leads to the speculation about the synergistic role of bipyridine exposure and the solar effect in causing these malignant and premalignant skin lesions".

Turmeric is a perennial plant in the genus Curcuma, belonging to the family Zingiberaceae, native to tropical South Asia.

The herb has been used in traditional medicine as an anti-oxidant, hypoglycemic, colorant, antiseptic, wound healing agent, and to treat flatulence, bloating, appetite loss, ulcers, eczema, inflammations, etc.

In the concerns of Ultraviolet B (UVB) irradiation in inducing skin photo-damage associated with the onset of a sunburn, erythema, photoaging, and photocarcinogenesis, researchers evaluated the protective effects of curcumin against acute photo-damage.

In hairless mice, topical application of curcumin exerted a kin-protective effect by significantly inhibiting acute UVB (540 mJ cm-2, for 3 successive days)-induced inflammatory cells, collagen accrementition derangement, and lipid peroxidation.

Additionally, the topical cream with curcumin also stimulated the NF-E2-related factor 2 (Nrf2) nuclear accumulation involved in skin defense on uncovered (Uncv) hairless mice skin.

In vitro, treatment of HaCaT cells with curcumin significantly attenuated acute UVB (300 mJ cm-2 ) tissue damage in releasing lactate dehydrogenase release (LDH) into the bloodstream.

Levels of intracellular reactive oxygen species production and DNA damage are also inhibited by the injection of curcumin by activating the expression of phase II detoxifying enzymes and promoting DNA repair activity.

In an additional analysis of the skin protective activity of curcumin, researchers found that The photoprotective effect provided by curcumin was potentially associated with modulation of the Nrf2-factor associated with antioxidant defense.

In order to reveal more information about the effects of turmeric anti skin damage, researchers looked into the turmeric and turmeric-containing polyherbal combination tablets versus placebo on facial redness.

The prospective, double-blind, randomized pilot study included 33 healthy participants who were recruited from the dermatology clinic at the University of California, Davis, and nearby community from 2016 to 2017 to receive one of three interventions (placebo, turmeric, or polyherbal combination tablets) and were told to take the intervention tablets by mouth twice daily for 4 weeks.

According to the results, the polyherbal combination group had a significant decrease in redness of 40% compared to baseline (P = 0.03) compared to that of placebo and turmeric groups with no statistically significant changes in image analysis-based facial redness.

Based on the findings, researchers said, "Polyherbal combination tablet supplementation improved facial redness compared to the turmeric or placebo".

Taken altogether, turmeric processed a high amount of bioactive compound curcumin used alone or combined with other phytochemicals may be used for the prevention of skin photo-damage, pending to the confirmation of the larger sample size and multicenter human study.

Intake of turmeric in the form of a supplement should be taken with extreme care to prevent overdose acute liver toxicity.

#Artichoke's Cynaropicrin Inhibits #ThyroidCancer Cell Proliferation through Cell Cycle Arrest, Scientists Reveal

Kyle J. Norton

Scientists may have found a herbal remedy for the treatment of thyroid cancer with no side effects, according to studies.
Thyroid cancer is a chronic medical condition characterized by irregular cell growth in the thyroid gland due to the alternation of cell DNA.

Most cases of thyroid cancer start in the cells on the surface of the inner lining of the gland, affecting the thyroid function in hormone production in the early stage.

About 45% of thyroid cancer is found in the population over the age of 45. According to the statistic, approximately over 53,000 adults in the United States are diagnosed with thyroid cancer every year.

In other words, the risk of thyroid cancer is about 4% a year, according to the data from 2005 to 2014. Overall, the 5-year survival rate for people with thyroid cancer is 98%.

The exact causes of thyroid cancer are unknown. Epidemiological studies suggested that people who have a family history of goiter, exposure to high levels of radiation, and certain hereditary syndromes are at an increased risk of the incidence.

Some researchers suggested that if you are female, your risk of thyroid cancer is increased substantially compared to your male counterpart.

Dr. Reza Rahbari, the lead scientist in the study to examine the thyroid cancer gender disparity, said, "Thyroid cancer is one of the fastest growing cancer diagnoses worldwide. It is 2.9 times more common in women than men".

And, "Recent studies on estrogen receptor status in thyroid cancer show a difference in the receptor subtypes expressed based on the histology of thyroid cancer. Moreover, the response to estrogen is dependent on the specific estrogen receptor expressed in thyroid cancer cells".

The results clearly suggested that the risk of thyroid cancer is 2.9 higher in females, probably due to the response to the hormone estrogen.

Artichoke is a perennial thistle of Cynara cardunculus species of the Cynara genus, belonging to the family Carduoideae native to Southern Europe around the Mediterranean.

The herbal plant has been used in traditional medicine as a liver protective and detoxified agent, and to treat digestive disorders, abdominal pain gas and bloating, etc.

Researchers on finding a natural chemo compound for the treatment of thyroid cancer examined the antithyroid cancer activity of the sesquiterpene lactone cynaropicrin, a major constituent of the artichoke leaves extracts.

The aim of the study was to investigate the effects of cynaropicrin, a major compound from the sesquiterpenes on the growth of three human thyroid carcinoma cell lines.

Injection of cynaropicrin was shown to inhibit the viability of CAL-62, 8505C, and SW1736 cells in a dose- and time-dependent way, with an EC50 of about 5 mM observed after 48 h of treatment.

In the cellular growth assays, cynaropicrin inhibited cell proliferation through the arrest of the cell cycle, mainly in the G2/M phase, observed by the presence of a significant percentage of necrotic cells.

Furthermore, the differentiation after 48 h of treatment with 10 mM of cynaropicrin suggested that the compound induced cell apoptosis also through the inhibition of the proteins associated with cell survival and cell cycle division.

Moreover, the application also reduced the free radical activity in exhibiting the expression of lipid peroxidation, without any significant effect on the reactive oxygen species production.

These results clearly suggested that cynaropicrin exhibited the function of free radicals in exerting the cytotoxic effect against thyroid cancer without causing harm to the healthy nearby cells through its antioxidant activity.

Taken altogether, artichoke processed abundantly bioactive compound sesquiterpene lactone cynaropicrin may be considered a functional remedy for the prevention and treatment of thyroid cancer, pending the confirmation of a large sample size and multicenter human study.

Formula #OptiBerry IH141, Including All Berries in the Studies Cuts Off the Energy Supply to #Cancer, Scientists Suggest

Kyle J. Norton

Bilberries may process a unique function in blocking the formation of new blood vessels to fuel tumor growth, according to studies.
Angiogenesis is the development of new blood vessels from the existing vasculature. The process has occurred throughout life in humans in the form of health and disease.

Normal angiogenesis ensures that developing or healing tissues receive an adequate supply of nutrients,

In a tumor, the availability of nutrients is limited, therefore, for the tumor to survive and grow continuously, the tumor cells induce the formation of a new blood supply from the preexisting vasculature by stimulating the protein associated with the pro-angiogenesis and facilitation of angiogenesis, such as vascular endothelial growth factor.

In other words, if we can cut off the blood vessels that provide nutrients to the tumor, we may cure cancers.

Dr. Adair TH, the lead scientist in the investigation of angiogenesis in diseases and cancer said, "Stimulation of angiogenesis can be therapeutic in ischemic heart disease, peripheral arterial disease, and wound healing. Decreasing or inhibiting angiogenesis can be therapeutic in cancer, ophthalmic conditions, rheumatoid arthritis, and other diseases".

And, "Capillaries grow and regress in healthy tissues according to functional dem bands. Exercise stimulates angiogenesis in skeletal muscle and the heart. A lack of exercise leads to capillary regression. Capillaries grow in adipose tissue during weight gain and regress during weight loss.

The results strongly suggested that by allowing or blocking the growth of new blood vessels, angiogenesis may be used as a primary target for the treatment of chronic diseases and cancers

Bilberry is a species of low-growing shrub in the genus Vaccinium, belonging to the family Ericaceae, native to Northern Europe.

The herbal plant has been used as foods and herbs in traditional medicine for the treatment of acute and chronic diarrhea, gastritis, gastric ulcer, duodenal ulcer, enterocolitis, ulcerative colitis, anemia, cystitis, kidney disease, psoriasis, diabetes, etc.

Researchers on finding a natural compound that blocks the progression of cancer examined the anti-angiogenic activity of berry anthocyanins.

Six berry extracts (wild blueberry, bilberry, cranberry, elderberry, raspberry seeds, and strawberry) included in the study were analyzed for the anti-angiogenic (the ability to reduce unwanted growth of blood vessels, which can lead to varicose veins and tumor formation) properties.

Formula OptiBerry IH141, including all berries used in the study, demonstrated superior anti-angiogenic properties compared to the other combinations tested.

OptiBerry significantly inhibited free radical and proinflammatory cytokines in the expression of VEGF (Vascular Endothelial Growth Factor) associated with the formation of new blood vessels.

According to the Matrigel assay using human microvascular endothelial cells, OptiBerry reduced the new blood vessel formation by impairing angiogenesis.

Futhermore, endothelioma cells pretreated with OptiBerry showed a diminished ability to form hemangioma and markedly decreased tumor growth by more than 50% through its anti-angiogenic, and anti-carcinogenic properties.

In order to reveal more information about berry's anti-angiogenic activity to prevent and treat cancer, researchers investigated the six berry extracts (wild blueberry, bilberry, cranberry, elderberry, raspberry seed, and strawberry) and a grape seed proanthocyanidin extract (GSPE) against the formation of new blood vessels for cancer proliferation.

Wild bilberry and blueberry extracts possessed the highest antioxidant activity observed by the ORAC values compared to other berries.

In other words, wild bilberry and blueberry extracts inhibited the overexpression of free radicals and proinflammatory cytokines in stimulating the production of VEGF (Vascular Endothelial Growth Factor) in enhancing angiogenesis.

Interestingly, these activities of wild bilberry and blueberry extracts were not shared by other antioxidants such as alpha-tocopherol or GSPE but were commonly shared by pure flavonoids.

Taken altogether, bilberry used alone or combined with other berries may be considered functional remedies for the prevention of angiogenesis and treatment of cancer cell proliferation by stimulating the formation of a new blood vessel, pending the confirmation of a large sample size and multicenter human study.

#Yoga Intervention Shows a Greater Decrease of Symptoms of #EatingDisorders Compared to Standard Care Patients, Researchers Say

By Kyle J. Norton

Yoga may be considered as an adjunct therapy combined with standard treatment for lower risk and treatment of eating disorders (EDs), a review study suggested.
Eating Disorders are a group of abnormal eating habits associated with a person's preoccupation with weight, involving either insufficient or excessive food intake.

Yoga therapy is an ancient practical technique of India, including breath control, meditation, bodily movement, and gesture,..... with an aim to achieve harmonization of the body, mind, and spirit.

In a systematic review of the literature that used yoga for preventing and treating EDs with a selection of 14 articles satisfied the guidelines, researchers found that most yoga practitioners indicated a decreased risk for eating disorders but symptoms were reduced or unchanged after yoga interventions.

Other, in the assessment of the effect of individualized yoga treatment on eating disorder outcomes among adolescents receiving outpatient care for diagnosed eating disorders (anorexia nervosa, bulimia nervosa, eating disorder not otherwise specified) of 50 girls and 4 boys aged 11-21 years randomized to an 8-week trial of standard care vs. individualized yoga plus standard care, researchers filed the following results

1. Yoga participants demonstrated greater decreases in eating disorder symptoms, according to Eating Disorder Examination(EDE)
2. Both groups maintained current BMI levels and decreased anxiety and depression over time.

3. Eating Disorder Examination(EDE) was back to baseline after the 12 weekly lessons.

Dr. Carei TR, the lead author said, "Individualized yoga treatment decreased EDE scores at 12 weeks, and significantly reduced food preoccupation immediately after yoga sessions". In other words, long-term yoga participation may be more effective in the treatment of an eating disorder.

Additionally, in the testing, the effects of yoga in decreasing risk factors, and increasing protective factors, for eating disorders, even with inconsistent results across studies researchers found that
1. Yoga lesson was a low-cost treatment program
2. Yoga may offer promise for the field of eating disorders, including
* Eating disorder prevention can be integrated into ongoing yoga classes
* Yoga can be integrated into an eating disorder prevention program with patients already in standard care

Dr. Neumark-Sztainer D said, "Regarding treatment, it is important to examine the effectiveness of different teaching styles and practices for different eating disorders".

Taken together, there is no doubt that yoga as a low-cost program may have a profound and positive effect in reducing the risk and treatment of the eating disorder. Unfortunately, most studies observed have weaknesses in their designs (e.g. observational design, no control groups, or small sample sizes) and are not consistent across studies, further studies, using stronger study designs, such as randomized, controlled trials, are needed.
