#Antioxidant #Bromelain Exhibits Enzymatic Debridement in #ChronicWound, Researchers Say

A chronic wound is a wound that does not heal within a predictable period of time.
It may be a result of the interruption of the stage of wound healing or damage of the blood vessels that have a function in providing necessary conditions to promote the closure of the wound as seen in a diabetic leg ulcer.

In other words, any wound that does not heal within 3 months is considered a chronic wound.

According to the statistics provided by the study "An Economic Evaluation of the Impact, Cost, and Medicare Policy Implications of Chronic Nonhealing Wounds", approximately, 2% of the population in developed countries suffer from chronic wounds.

Additionally, every year, more than 305 million cases of acute, traumatic, and burn wounds are treated globally.

Believe it or not, chronic and acute wound affects more than nine times of people around the world compared to those living with cancer.

Some researchers suggested that pressure, trauma, and/or lower extremity wounds, increased bacterial load, interruption of the stage of wound healing including excessive proteases, degraded cell surface structures, and inappropriate treatment are considered the most potent causes of the conditions.

More precisely, if we can protect all stages of wound healing without interruption, the chronic wound can be preventable.

Bromelain, a proteolytic enzyme found in pineapples (Ananas comosus) has been used in traditional medicine as an inflammatory agent for the treatment of pains, strains, muscle aches, and pains and to ease back pain and chronic joint pain, skin diseases, etc.

With an aim to find a natural compound for the treatment of chronic wounds, researchers examined the efficacy of bromelain-based enzymatic debridement of chronic wounds.

The study included 24 patients with chronic wounds of different aetiologies.

According to 2 consecutive single-arm studies assessing the enzymatic debridement efficacy in 2 medical centers, all wounds achieved an average of 68% ± 30% debridement in an average of 3.5 ± 2.8 enzymatic debridement 4-hour sessions.

17/24 patients showed responding wounds (venous, diabetic, pressure, and post-traumatic aetiologies) of an average of 85% ± 12% debridement in 3.2 ± 2.5 applications, compared to 7/24 patients with no response.

There were no adverse effects reported by the patients. Based on the findings, researchers wrote, "These preliminary results indicate the potential safety and efficacy of bromelain-based enzymatic debridement in chronic wounds".

In vitro, according to the results of two soft tissue healing--fibroblasts and microvascular endothelial cells--were used as mono- and co-culture with bromelain, bromelain attenuated endothelial cell, and fibroblast proliferation moderately.


The inhibition of cell proliferation was not associated with cell apoptosis but with cell cycle arrest in the G0 phase.

In the comparable test, bromelain did not reduce endothelial cell migration. whereas fibroblast migration was clearly slowed down.

In other words, bromelain displayed a significant decrease of myofibroblasts under both normoxic (from 19 to 12 %) and hypoxic conditions (from 22 to 15 %) coincident with higher levels of anti-inflammatory expression.

Based on the findings, researchers wrote, "Usage of bromelain as a therapeutic drug for chronic human wounds thus remains a very promising concept for the future".

Taken altogether, bromelain used alone or combined with other remedies may be considered supplements for the prevention and treatment of chronic wounds, pending the confirmation of the larger sample size and multicenter human study.

Intake of bromelain in the form of supplements should be taken with extreme care to prevent overdose acute liver toxicity.

Cancers are a group of chronic and medical diseases characterized by irregular cell growth in the body tissue caused by the change of DNA transcription of healthy cells.

Normal cells have the division of a limited cell cycle compared to a cancerous cell which can grow unlimitedly.

If an invasive tumor is growing too large, it can suppress the nerves and muscles surrounding them, leading to persistent pain and other symptoms, depending on the location.

The exact causes of cancer are unknown. Researchers do not know why people with the same health condition, are susceptible to the onset of the diseases, while others do not.

However, an unhealthy diet, such as eating processed foods and fast foods, and lifestyles such as smoking, being overweight, physical inactivity, and excessive alcohol drinking are some of the major causes of cancer.

Based on the findings, researchers wrote, "Curcumin and also attributes towards probable mechanistic insight into its therapeutic potential as a cardio-protective agent for regression of cardiac hypertrophy".

Hypertension, found in the cluster of metabolic syndrome is a medical condition associated with abnormally high blood pressure.

Blood pressure change constantly depending on the activity. Persistent hypertension over time can lead to diabetes and cardiovascular disease.

It also is the force of blood pushing against the walls of the arteries as the heart pumps out blood.

According to the statistics provided by the American Heart Association., approximately 103 million U.S. adults are living with high blood pressure, accounting for nearly half of all adults in the United States.

Conventionally, blood pressure medications have been found to prevent many deaths from heart disease and stroke over the past 30 years, however, long-term intake of such medications has caused many side effects such as damaging the kidney.

Epidemiologically, family history, aging, being male, being overweight or obese, ethnicity, physical inactivity, unhealthy diet, and people with chronic kidney disease are some common risk factors involved in the increased risk of hypertension.

Believe it or not, excessive alcohol drinking is also one of the major causes besides obesity in elevating the syndrome onset.

Dr. Kazim Husain wrote, "Several possible mechanisms have been proposed such as an imbalance of the central nervous system, impairment of the baroreceptors, enhanced sympathetic activity, stimulation of the renin-angiotensin-aldosterone system, increased cortisol levels, increased vascular reactivity due to increasing in intracellular calcium levels, stimulation of the endothelium to release vasoconstrictors and loss of relaxation due to inflammation and oxidative injury of the endothelium leading to inhibition of endothelium-dependent nitric oxide production".

The results strongly showed support for the use of Lichtwer garlic powder (containing 1.3% alliin equivalent to 0.6 % allicin) for the treatment of systolic blood pressure in hypercholesterolemic in an animal model.

Dr. Ali M the lead scientist said, "Our experimental results show that garlic may beneficially affect .....hypertension".

Taken altogether, alliin may be considered a supplement for the prevention and treatment of hypertension, pending the confirmation of the larger sample size and multicenter human study.

In the study of Anise estrogenic activity at doses levels of 50 mg, 70 mg, and 80 mg/kg PO. in rats, researchers found that application of Anise at all doses exerted a strong estrogenic effect without affecting the levels of progestational, anti-progestational, androgenic or anti-androgenic activities and inducing any side effects even at the doses of more than 3000 mg/kg with LD50, an indication of 50% percent of a population of animals killed usually within a certain time due to some application in a dose-dependent manner.

Also, in the investigation of the Anise estrogenic effect, researchers found that the application of Anise displayed estrogenic activity through increased milk secretion, promoting menstruation, facilitating birth, alleviating the symptoms of the male climacteric, and increasing libido.

Further analysis, also suggested that the implication of fennel estrogenic activity was associated with the major bioactive compound anethole found at high levels in the fennel essential oil.

However, even with a lesser amount in fennel essential oil, polymers of anethole, the derivative of di anethole, and photo anethole are considered actual pharmacological values in testing for the drug in the treatment of estrogenic deficiency-related diseases, including menstrual disorder, libido, menopause symptoms, male climacteric,.....

The above differentiation was supported by the evaluation of the estrogenic activity of essential oils of eleven Pimpinella species and the compounds isolated from these species, conducted by Anadolu University.

According to the yeast estrogen screen (YES) assay in the study, Anise essential oils containing (E)-anethole as major compounds showed estrogenic activity, compared to insignificant expression in aerial parts without fruits of P. anisetum and P. flabellifolia.

In the presence of anethole-containing oils from 30-50%, Anise exerted the maximal response in expression estrogenic activity.

Interestingly, Fruits of P. isaurica and P. peucedanifolia were estrogenic in spite of the absence or trace amount of anethole.

These results suggested that the estrogenic activity of Pimpinella oils may also be contributed by other components other than anethole.

In other words, anethole, a major bioactive compound may not be solely responsible for contributing towards estrogenic activity and essential oil isolated from different parts of the plant and species expressed a varied in estrogenic potencies compared to 17 beta-estradiol.

In herbal Anise, the fruit oils were most estrogenic followed by the aerial parts without fruits and the root oils and their EC50 values varied from 45 micrograms/mL to 650 micrograms/mL.

Taken together, Anise and its major bioactive compound anethole may be considered as a functional food for reduced risk, progression, and treatment of estrogenic deficiency syndrome and diseases, such as menopause symptoms and women's reproductive diseases.

Acute inflammatory inflammation results from the over-response of the immune system after sensing the injury or damage of the body by activating the protein associated with the production of pro-inflammatory cytokines.

The aim of the proinflammatory cytokines is to kill the invasive pathogens, which try to enter the body through the wound.

In the acute phase of infection, the immune system in the stimulating production of proinflammatory cytokines may induce overproduction of these cytokines can cause death or damage to the nearby cells, leading to scar formation and symptoms of localized swelling, redness, and fever.

Chronic inflammation is a serious condition caused by the immune system's to adapt to the new change due to its inability to kill all the invasions within a set of times.

Vasoprotection is the process to improve the function of the blood vessels, including the use of herbal and natural remedies.

Blood vessels are the tubular structure including a vein, artery, or capillary which carries the blood through the tissues and organs pumped by the heart.

On findings, a potential compound for the treatment of acute vascular inflammation researchers evaluated the anti-inflammatory activity, and the cytotoxic effect of a novel chemically modified, non-carbonyl compound enriched Curcuma longa L. (C. longa) extract (CMCE).

According to the tested analysis, CMCE at the concentration of 1 or 10 μg/mL at 14 h, significantly decreased LPS (50-100 ng/mL) induced production of pro-inflammatory cytokines including TLR4 expression in THP-1 cells, human, and mouse whole blood.

In THP-1 cells, CMCE inhibited the expression of IRAK1, which plays a critical role in initiating an innate immune response against foreign pathogens, MAPK is involved in directing cellular responses to a diverse array of stimuli.

CMCE (30, 100, and 300 mg/kg; 10 days p.o.) pre-treated and LPS (10 mg/kg) in Swiss mice exhibited attenuated plasma TNF-α, IL-1β, nitrite, aortic iNOS expression, and vascular dysfunction associated with the production of pro-inflammatory cytokines.

In HL-60 cells., CMCE. exerted cytotoxic effects through cell cycle arrest on the Sub-G1 phase, increased apoptotic cells, loss of mitochondrial membrane in the cancer cells, and activation of cascade associated with pro-apoptosis.

Taken altogether, turmeric processed abundantly bioactive compound curcumin may be considered supplements for the prevention and treatment of vascular inflammation, pending the confirmation of the larger sample size and multicenter human study.

Intake of turmeric in the form of supplements should be taken with extreme care to prevent overdose acute liver toxicity.
