Ulcerative colitis is a condition of inflammatory bowel disease (IBD) inducing long-lasting inflammation and ulcers in the digestive tract.

In a randomized, double-blind, placebo-controlled trial in evaluating the efficacy of A. paniculata extract (HMPL-004) in 224 adults with mild-to-moderate ulcerative colitis. with patients randomized to A. paniculata extract (HMPL-004) 1,200 mg or 1,800 mg daily or placebo for 8 weeks, patients treated with A. paniculata extract (HMPL-004) at a dose of 1,800 mg daily achieve clinical response than those receiving placebo.

The study also discovered that 60 and 53% of patients treated with A. paniculata groups experienced adverse effects at doses of 1,200 mg and 1,800 mg daily respectively in comparison to 60% in the placebo group.

Dr. Sandborn WJ, the lead author in the study said, "Andrographis paniculata has in vitro inhibitory activity against TNF-α, IL-1β, and NF-κB. A pilot study of A. paniculata extract (HMPL-004) suggested similar efficacy to mesalamine for ulcerative colitis."

Another study involved herbal extract HMPL-004 also supports the use of herbal extract for treatment in patients with mild-to-moderate ulcerative colitis, according to the randomized, double-blind, multicentre, 8-week parallel-group study conducted using HMPL-004 with 1200 mg/day in compared with 4500 mg/day of slow-release mesalazine (mesalamine) granules in the same group.

In fact, in comparison to mesalazine, HMPL-004 exhibited a 21% and 76% of more efficacy in treated patients, compared to 16% and 82% of mesalazine-treated patients through the colonoscopy, remission, and response.

Although HMPL-004 may be an efficacious alternative to mesalazine in treating ulcerative colitis, the herbal medicine can only be prescribed by herbalists to prevent overdose toxicity.

#Herbal #Bilberry Extracts Inhibit the Risk of Colon Cancer, Researchers Show

Colon cancer is a medical and chronic condition characterized by the growth of cells disorderly and uncontrollably in the colon tissue.
Most cases of colon cancers begin in the cells on the surface of the inner lining due to the alternation of DNA.

Colon cancer is one most slow-growing cancer. It may take years before patients experience the symptoms.

At the later stage, the larger size of the tumor may suppress the nearby blood vessels and nerve cells, leading to localized severe pain and bleeding.

The most common symptoms of colon cancer in the advanced stage, besides the general symptom of cancer such as reduced appetite, and unintended weight loss include a change of bowel movement, rectal bleeding, and persistent abdominal pain and cramp.

According to the statistics, in 2017, approximately 26,800 Canadians were diagnosed with colorectal cancer, representing 13% of all new cancer cases.

The herbal plant has been used as foods and herbs in traditional medicine for the treatment of acute and chronic diarrhea, gastritis, gastric ulcer, duodenal ulcer, enterocolitis, ulcerative colitis, anemia, cystitis, kidney disease, psoriasis, diabetes, etc.

Researchers on finding a natural compound for the treatment of colon cancer examined the bilberry-derived anthocyanins' chemopreventive properties.

The study included 50 female Balb/c mice induced colonic carcinogenesis by AOM and DSS 3 or 5%, respectively, Mice then are given either normal food (controls) or a diet containing either 10 or 1% anthocyanin-rich bilberry extract.

According to the colonoscopy taken place at weeks 4 and 9 after initiation of carcinogenesis, mice receiving 10% anthocyanins showed significantly (p < 0.004) less reduced colon length (12.1 cm [8.5-14.4 cm]) as compared to controls (11.2 cm [9.8-12.3]) indicating less inflammation.

Furthermore, mice fed with 10% anthocyanin-rich extract revealed significantly less mean tumor numbers, compared to control (n = 14) and anthocyanin 1% treated mice.

The results suggested that anthocyanins inhibited the formation and growth of colorectal cancer in an animal model.


In order to reveal more information about anthocyanins' anti-colon cancer activity, researchers examined the compound on human colon and colorectal cancer cells (DLD-1 and COLO205).

Using the optimum process of A(V.uli) extraction involved conditions of temperature 20°C, pH 2.0, and diatomaceous earth 1.0 g/50 g of fruit weight. A(V.uli), 5 main components: delphinidin (40.70 ± 1.72)%, cyanidin (3.40 ± 0.68)%, petunidin (17.70 ± 0.54)%, peonidin (2.90 ± 0.63)% and malvidin (35.50 ± 1.11)% were found in the anthocyanins extract.

Malvidin, the derivatives found in the extracts exerted the strongest anti-colon cancer cell properties with dose-dependent repression of cancer cell proliferation with an IC(50) (50% inhibitory concentration) value of 50 µg/ml, compared to other derivatives.

In the final report, Dr. Zu XY and colleagues wrote, "The extraction protocol and conditions we used were effective for anthocyanin extraction. A(V.uli) could be a feasible practical research tool and a promising therapeutic source to suppress human colon or colorectal cancers".

Taken altogether, bilberry-processed abundantly bioactive compounds anthocyanin and malvidin may be considered a functional remedy for the prevention and treatment of colon cancer, pending the confirmation of a larger sample size and multicenter human study.

Non-alcoholic liver disease is a medical condition caused by excessive fat accumulated in the liver of people who drink little or no alcohol.

Believe it or not, non-alcoholic fatty liver disease (NAFLD) now becomes a universal disorder and the most common liver disease in the Western world.

According to the statistics in 2015, approximately, 20 to 30% of the general population in the Western world suffer from NAFLD. The prevalent rate is substantially higher in the population with type 2 diabetes mellitus (70%) and morbid obesity (90%).

Some researchers in the assessment of the relationship of widespread obesity in the risk of NAFLD suggested that non-alcoholic fatty liver disease may be caused by the promotion of the Western diet over the past few decades.

Dr. Kristen Stephenson, in the assessment of the impact of dietary fat on the development of non-alcoholic fatty liver disease in the animal model, wrote, "WD mouse models consisting of high fat, cholesterol, and a combination of high-fructose corn syrup, sucrose, fructose, or glucose not only lead to metabolic syndrome but also induce NAFLD with fibrosis".

Artichoke is a perennial thistle of Cynara cardunculus species of the Cynara genus, belonging to the family Carduoideae native to Southern Europe around the Mediterranean.

The herbal plant has been used in traditional medicine as liver protective and detoxified agents, and to treat digestive disorders, abdominal pain gas and bloating, etc.

Researchers on finding a natural therapeutic for the treatment of Non-alcoholic fatty liver disease (NAFLD), the most common cause of chronic liver disease worldwide investigated the artichoke leaf extract (ALE) therapeutic utility in patients with established NAFLD.

The randomized double-blind placebo-controlled parallel-group trial included 100 subjects with ultrasound-diagnosed NAFLD randomized to either ALE 600 mg daily or placebo for a 2-month period.
Compared to the control, the ALE treatment group showed increased hepatic vein flow, reduced portal vein diameter and liver size, according to the Doppler sonography. In other words, ALE treatment improved liver function.

Inflammatory and oxidative stress such as serum ALT, AST levels, and total bilirubin were also decreased upon the injection of ALE observed by the improved AST/ALT ratio.

Furthermore, ALE supplementation also reduced total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, non-high-density lipoprotein cholesterol, and triglyceride concentrations. All these parameters were elevated in patients with NAFLD.


Dr. Panahi Y, the lead scientist, after taking into account other factors, wrote, " This study has shown beneficial effects of ALE supplementation on both ultrasound liver parameters and liver serum parameters (ALT, AST, APRI ratio, and total bilirubin) in patients with NAFLD".

In order to reveal more information about artichoke anti-NAFLD activity, researchers elucidated the physiological responses and mechanisms of JA intervention with comprehensive transcriptome analysis.

The study included Wistar rats fed a control diet, a 60 % fructose-enriched diet (FRU), or an FRU with 10 % JA (n 6-7) for 4 weeks with an oral glucose tolerance test carried out on day 21.


According to the assays, dietary JA supplementation improved the IR and hepatic TAG accumulation induced by Fructose feeding.

JA also inhibited the gene expression of malic enzyme 1 (Me1), associated with fatty acid synthesis; decorin (Dcn), related to fibrosis; and cytochrome P450, family 1, subfamily a, polypeptide 2 (Cyp1a2) and nicotinamide phosphoribosyltransferase (Nampt), associated with inflammation.


Based on these results, researchers proposed that 10 % of JA supplementation may be beneficial for the prevention of the onset of NAFLD.

Taken altogether, artichoke may be considered a functional remedy for the treatment of NAFLD with no side effects, pending the validation of a larger sample size and multicenter human study.

Ginger may have a potential and profound effect in regulating abnormally high blood sugar, some scientists suggested.

Hyperglycemia is a condition of abnormally high levels of glucose in the bloodstream.

According to the statistic, approximately, 30.3 million Americans, or 9.4% of the population, had diabetes. Approximately 1.25 million American children and adults have type 1 diabetes. And, hyperglycemia is a hallmark sign of diabetes(both type 1 diabetes and type 2 diabetes) and prediabetes.

Some researchers suggested that people who are living under prolonged periods of stress due to chronic illness or stress are associated with a higher risk of high blood sugar.

Others suggested that an unhealthy diet such as high in saturated and trans fat, red meat, and processed food, and lack of physical exercise have many folds increased the risk for the onset of hyperglycemia.

In fact, shift workers accompanied by an unhealthy lifestyle are the joint associated with the risk of type 2 diabetes.

Dr. Shan Z, in the concerns of the rotating night shift work and lifestyle factors in the risk of diabetes, used the data of Nurses' Health Study (1988-2012) and Nurses' Health Study II (1991-2013) and compared to the 143 410 women without type 2 diabetes, cardiovascular disease, or cancer at baseline, said, " The multivariable-adjusted hazard ratios for type 2 diabetes were 1.31 (95% confidence interval 1.19 to 1.44) per five-year increment of a duration of rotating night shift work and 2.30 per unhealthy lifestyle factor (ever smoking, low diet quality, low physical activity, and overweight or obesity)".

"For the joint association of per five-year increment rotating night shift work and per unhealthy lifestyle factor with type 2 diabetes, the hazard ratio was 2.83 (2.15 to 3.73) with a significant additive interaction (P for interaction <0.001)".

How severe is the diabetes risk in nurses on the rotating shift accompanied by an unhealthy diet? According to the team, during 22-24 years of follow-up, 10 915 cases of incident type 2 diabetes occurred.

The risk of type 2 diabetes is even higher if each individual factor is also included.

These results clearly suggested, the onset of diabetes is not caused by a single factor but is associated with a group set of risk factors.


Luckily, most people with some of the above risk factors do not develop hyperglycemia or type 2 diabetes.

Ginger (Zingiber officinale) or ginger root, the second superfood used for thousands of years by mankind, is the genus Zingiber, belonging to the family Zingiberaceae, native to Tamil.

The root has been used in traditional and Chinese medicine to treat dyspepsia, gastroparesis, constipation, edema, difficult urination, colic, etc.

On finding a natural ingredient and whole food for the treatment of diabetes and diabetes complications, researchers examine the action of ginger in the diabetic population.

Selected mice fed by either a high-fat diet or a standard diet for a period of 17 weeks were given phloretin isolated from apples and [6]-gingerol isolated from ginger supplemented at two different doses.

Application of phloretin and [6]-gingerol inhibited the formation of AGEs by suppressing the receptor for advanced glycation end products (RAGE).

Where Advanced glycation end products are proteins or lipids that become glycated as a result of exposure to sugars. The products can substantially worsen certain chronic diseases such as diabetes.

Phloretin or [6]-gingerol supplementation significantly reduced plasma glucose, alanine aminotransferase, aspartate aminotransferase, AGEs, and insulin levels and enhanced GSH/GSSG ratio.

These results suggested that phloretin or [6]-gingerol supplementation not only inhibited the oxidative stress that causes abnormally high blood glucose but also improved insulin resistance against the onset of diabetes complications in mice fed with a high-fat diet.

Furthermore, the assessment of the effect of ginger consumption on glycemic status, lipid profile, and some inflammatory markers in patients with type 2 diabetes mellitus with the consumption of 1600 mg ginger versus 1600 mg wheat flour placebo daily for 12 weeks, also found that ginger reduced fasting plasma glucose, HbA1C, insulin, HOMA, triglyceride, total cholesterol, CRP and PGE₂ significantly compared with the placebo group (p < 0.05).

Ginger improved fasting glucose and parameters associated with the onset of insulin resistance without inducing any side effects.

Dr. Arablou T after taking into account co and confounders said, "Ginger improved insulin sensitivity and some fractions of lipid profile, and reduced CRP and PGE₂ in type 2 diabetic patients. Therefore ginger can be considered an effective treatment for the prevention of diabetes complications".

Taken altogether, ginger may be considered a functional food for the prevention and treatment of diabetes and diabetes complications induced by abnormally high blood glucose.

Antioxidants are stable molecules with a unique function in the inhibition of oxidation.
Oxidative stress is a condition caused by an imbalance of the ratio of free radicals and antioxidants in the body.

In other words, oxidative stress is either caused by overexpression of free radicals or depletion of antioxidant enzymes in the body.

Some researchers suggested that oxidative stress may also arise due to the chain reaction of free radicals in which free radicals donate or intercept an electron from other stable molecules.

This chain of reaction is the cause of oxidative stress, a condition is associated with either overexpression of free radicals or depletion of antioxidant enzymes produced by the host organs.

Overexpression of free radicals is known to cause damage in contributing to the etiology of many chronic health problems such as cardiovascular and inflammatory disease, cataracts, and cancer.

Long-term oxidative stress present in the body can cause protein and lipid damage and alternation of cell DNA, leading to the onset of chronic diseases, including cardiovascular disease, diabetes, and cancer.

More precisely, exogenous factors, such as cigarette smoke, and endogenous factors, such as the oxidative burst from activated macrophages are some of the factors involved in the risk of oxidative stress.

Free radicals are highly reactive molecules with an unpaired electron on the outer ring of the atom.
Due to their nature to become stable, free radicals readily donate or steal an electron from other molecules, leading to the chain of domino effect that can not be stopped until either the electrons are paired or inhibited by antioxidants.

Parkinson's disease is a progressive neurodegenerative disease that affects movement caused by the gradual death of neurons that produce a chemical messenger in the brain called dopamine.

* Pretreatment of SH-SY5Y cells with dithiole-4-carboxylic acid ethyl ester (ACDT), a derivative of Dithiolethiones induced the production of the levels of the antioxidant glutathione (GSH) in a time-concentration-dependent manner by inhibiting the GSH-synthesizing enzyme glutamate-cysteine ligase catalytic subunit (GCLC).

* Furthermore, the application of ACDT also promoted the expression of antioxidant through the activity of nuclear factor erythroid-2 related factor (Nrf2), which protect the cells against oxidative damage triggered by injury and inflammation.


Based on the findings, researchers wrote, "Collectively, the data demonstrated ACDT to be a promising new dithiolethione for the treatment of PD, with two modifiable functional groups offering additional avenues for enhanced pharmacological application",

The Effects of Phytochemical 3,3'-Diindolylmethane on the Delayed-Type Hypersensitivity

Delayed-type hypersensitivity is also known as type 4 hypersensitivity because the reaction takes several days to develop, including contact dermatitis.

Believe it or not, type IV hypersensitivity mediated by antigen-specific effector T cells is associated with the infection induced by slow-growing intracellular organisms, such as M. tuberculosis (tuberculosis), M. leprae (leprosy), and H. capsulatum.

Some researchers suggested that the delayed hypersensitivity is dependent on the presence of a significant number of primed, antigen-specific T cells response which typically reaches its peak 24 to 48 hours after exposure to antigen.

Type IV hypersensitivity diseases although common, range second to type 1 hypersensitivity.

Many diseases are found to associate with delayed-type hypersensitivity including leprosy, tuberculosis, schistosomiasis, sarcoidosis, and Crohn's disease.
* Leprosy is divided clinically into three essential types: tuberculoid, borderline, and lepromatous.

* Schistosomiasis is caused by schistosomes, once the body is sensitized by a granulomatous reaction developed in the parasitized tissue mediated by Th2 lymphocytes.

* Tuberculosis in the lungs is caused by a granulomatous reaction that leads to cavitation and the spread of bacteria

* Sarcoidosis is a chronic and idiopathic disease caused by the activated macrophage and granuloma accumulation in many tissues along with fibrosis.


* Crohn's disease is a granulomatous disease that is not caused by a microorganism.


3,3'-Diindolylmethane or DIM are phytochemicals derived from the digestion of indole-3-carbinol, belonging to the group of Indoles, found abundantly in broccoli, Brussels sprouts, cabbage, and kale, etc.

In finding potential ingredients for the prevention and treatment of delayed-type hypersensitivity, researchers tested the ability of dietary AhR ligands (indole-3-carbinol [I3C] and 3,3'-diindolylmethane [DIM]) and an endogenous AhR ligand, 6-formylindolo(3,2-b)carbazole (FICZ), on the differentiation and functions of Tregs and Th17 cells.

Administration of indoles (I3C or DIM) in tested mice attenuated delayed-type hypersensitivity (DTH) response to methylated BSA, an antigen-specific inflammation in targeted organs and generation of Th17 cells which play a critical role in the induction of the tissue inflammation and tissue destruction while promoting Tregs in the suppression of immune response.

Interestingly, the indoles inhibited the delayed-type hypersensitivity by causing the reciprocal induction of Tregs and Th17 cells only in wild-type (AhR(+/+)) but not in AhR knockout (AhR(-/-)) mice.

In an additional analysis of the indole's anti-delayed-type hypersensitivity activity, researchers found that the efficacy of the indoles was associated with the promotion of protein (FoxP3) involved in immune system responses of the Tregs while decreasing the protein associated with pro-inflammatory expression.

In order to reveal more anti-inflammation about indole-3-carbinol (I3C) and diindolylmethane (DIM), researchers investigated the compounds in the exhibition of the characteristics of aryl hydrocarbon receptor (AhR) ligands, that were found to regulate the T cell differentiation.

In a murine model of multiple sclerosis (MS), retreatment with I3C or DIM completely prevented the clinical symptoms and cellular infiltration into the CNS through the induction of inflammation.

Particularly, post-treatment of EAE with I3C or DIM proved highly effective in curtailing the overall severity of the disease.

Similar to that of the aforementioned protection against delayed-type hypersensitivity, I3C or DIM promoted the generation of T-regs, while inhibiting the induction of MOG-specific Th17 cells.

In other words, I3C or DIM inhibited delayed-type hypersensitivity through the promotion of T-regs and suppression of Th17 cells in vivo and in vitro were found to be AhR-dependent.

Taken altogether, 3,3'-Diindolylmethane derived from the digestion of indole-3-carbinol may be considered a supplement for the prevention and treatment of delayed-type hypersensitivity, pending to the confirmation of the larger sample size and multicenter human study.

#Ashwagandha in the Promotion of #MaleFertility, According to Researchers

Fertility is a natural process to ensure the survival of the human species. Through natural selection, we produce many offsprings when the reproductive system works at its peak in a suitable environment.
In other words, female fertility is the ability of women to conceive a biological child before menopause compared to male fertility which generally relies on the quantity and quality of the sperm.

But regardless of all factors, most women are capable of conceiving during their menstrual cycle before reaching the stage of menopause.

Infertility is a condition characterized by the inability of a couple to conceive after 12 months of unprotected sexual intercourse or can not carry the pregnancy to term.

The condition affects over 5 million couples alone in the U. S. and many times more in the world. Because of unawareness of treatments, only 10% seek help from a professional specialist.

Male infertility is a medical condition associated with the male's inability to cause pregnancy in a fertile female.

Male infertility accounts for 40–50% of all cases of infertility, affecting over 7% of all men.
Most cases of male infertility are the results of deficiencies in the semen, and semen quality that lead to reduced function of male fecundity.


The exact causes of male infertility are not identified. However, abnormal sperm production or function due to genetic defects, and medical conditions such as diabetes, or infectious diseases such as chlamydia, gonorrhea, mumps are some of the prevalent factors that are involved in the increased risk of male infertility.

Believe it or not, stress caused by a number of health problems, including heart disease, asthma, obesity, and depression may lead to reduce sperm and semen quality in male fertility.

Dr. Nargund VH said, "Psychological stress can be defined as any uncomfortable 'emotional experience' accompanied by predictable biochemical, physiological, and behavioral changes or responses. Many clinical studies looking at the effects of psychological stress on male fertility have shown that stress is associated with reduced paternity and abnormal semen parameters".

And, "psychological stress could severely affect spermatogenesis, mainly as a result of varying testosterone secretion. The hypothalamic-pituitary-adrenal axis has a direct inhibitory action on the hypothalamic-pituitary-gonadal (HPG) axis and Leydig cells in the testes. The newly discovered hormone, gonadotropin-inhibitory hormone (GnIH), also has an inhibitory effect on the HPG axis. Inhibition of the HPG axis results in a fall in testosterone levels, which causes changes in Sertoli cells and the blood-testis barrier, leading to the arrest of spermatogenesis".

Ashwagandha also known as Withania somnifera is a nightshade plant in the genus of Withania, belonging to the family Solanaceae, native to the dry parts of India, North Africa, the Middle East, and the Mediterranean. The herbal medicine has been considered as Indian ginseng and used in Ayurvedic medicine for over 3000 years to treat tumors and tubercular glands, carbuncles, memory loss, and ulcers and considered an anti-stress, cognition-facilitating, anti-inflammatory, and anti-aging herbal medicine.

In finding a potential compound for the improvement of male fertility, researchers examined the efficacy and safety of W. somnifera treatment in infertile men.

The evidence-based systematic review and meta-analysis were conducted by searching the database of medical literature PubMed/MEDLINE, EMBASE, Scopus, the Cochrane Library, and DHARA. Grey literature was assessed from the WHO International Clinical Trials Registry Platform (http://apps.who.int/trialsearch/) and the US National Institutes of Health (https://clinicaltrials.gov/).

According to the 4 clinical trials (comprising 5 publications: observational, n = 4; randomized controlled trial [RCT], n = 1) only one RCT was included in the analysis, W. somnifera treatment significantly improved semen parameters including semen volume: mean difference [MD], 0.28  ml; sperm concentration: MD, 13.57 million/ml; sperm motility: MD, 8.50%

Furthermore, W. somnifera administration also improves the fertility rate in normozoospermic men by 14%

Moreover, according to the results of the meta-analysis, W. somnifera treatment also exerted a significant improvement in serum hormonal profile, oxidative biomarkers, and antioxidant vitamins in seminal plasma.

No adverse effects were reported during the study.

In other words, W. somnifera enhances all parameters associated with sperm integrity and viability in the promotion of conception without inducing any side effects.

Based on the findings, researchers after taking other factors into account wrote, "Due to a small number of eligible studies, the available data, though promising, are too limited to provide novel and sufficiently robust evidence of the benefits of W. somnifera in male infertility. Additional RCTs of high quality with a larger sample size are warranted to further strengthen clinical use of W. somnifera in treating male factor infertility".


Taken altogether, Ashwagandha may be considered a remedy for the treatment of male infertility, pending the confirmation of the larger sample size and multicenter human study.
